摘 要:目的:通过CT灌注成像结果与非酒精性脂肪肝病实验兔的病理结果进行对照分析,探索CT灌注定量参数在非酒精性脂肪肝病的变化。方法:通过高脂高胆固醇饮食的方法,制作实验兔的非酒精性脂肪肝病模型。成模后将实验兔行CT灌注扫描,计算CT灌注参数血流量(BF)、血容量(BV)、肝动脉灌注(ALP)、门静脉灌注(PVP)、肝动脉灌注指数(HPI),并与病理结果进行对比。结果:高脂饮食的实验兔全部发展为非酒精性脂肪肝病,正常组为9只,可疑非酒精性脂肪肝炎组(Borderline组)13只,非酒精性脂肪肝炎组(NASH组)19只。不同组间(正常对照组与Borderline组、NASH组)肝脏CT值、肝脏/竖脊肌CT比值差异有统计学意义(P=0.000),各组间均差异有统计学意义,NASH组<Borderline组<正常对照组(P均<0.05)。不同组间的ALP差异无统计学意义(P=0.432),BF、BV、PVP及HPI差异有统计学意义(P值分别为0.017,0.003,0.002和0.000),Borderline组的BV较正常对照组减低(P<0.05),Borderline、NASH组的BF、PVP较正常对照组减低,HPI升高,差异均有统计学意义(P均<0.05),但Borderline组和NASH组的BF、BV、PVP、HPI差异均无统计学意义(P均>0.05)。结论:非酒精性脂肪肝病实验兔的肝脏CTP中的BF、BV出现下降,门脉灌注量PVP明显下降,同时出现HPI增高,提示NAFLD微循环灌注下降后,可出现类似于肝硬化的肝动脉缓冲效应。Purpose: To evaluate the changes of CT perfusion (CTP) parameters in nonalcoholic fatty liver disease(NAFLD) rabbit model. Methods: The rabbit NAFLD model was duplicated by means of high fat diet. CT perfusion study was performed on each rabbit. The CT perfusion parameters including BF, BV, ALP, PVP, and HPI were calculated and compared with pathology results. Results:All rabbits given high fat diet developed NAFLD. The final cohort consisted of normal group (n=9), borderline group (n= 13), and NASH group (n=19). The CT value of liver, CT value ratio of liver/muscle were with differences among three groups (P=0.000), NASH group〈Borderline group〈normal group (P〈0.05). There was no difference of ALP among three groups (P=0.432). BF, BV, PVP, and HPI were found with differences among three groups (P=0.017,0.003,0.002, and 0.000, respectively). BV of borderline group was less than that of normal group (P〈0.05). BF, PVP of borderline and NASH group were less than those of normal group, while HPI was larger than that of normal group (P〈0.05). No difference of BF, BV, PVP, and HPI were found between borderline and NASH group.Conclusion: The CTP parameters of BF and BV were decreased, PVP was decreased significantly, while HPI was increased in NAFLD rabbit model, which indicate microcirculation disturbance in NAFLD and hepatic arterial buffer response as a consequence like cirrhosis.
