养阴清肺汤联合西药治疗老年慢性阻塞性肺气肿(阴虚燥热)随机平行对照研究

摘　　要：[目的]观察养阴清肺汤联合西药治疗老年慢性阻塞性肺气肿(阴虚燥热)疗效。[方法]使用随机平行对照方法,将90例住院患者按病志号抽签方法随机分为两组。对照组45例氨茶碱0.1~0.2g/次,3次/d;沙美特罗替卡松,2次/d,雾化吸入;左氧氟沙星0.5g/次,1次/d。治疗组45例养阴清肺汤(白芍、地黄^(生)各15g;牡丹皮、^(川)贝母、麦冬、玄参各10g;甘草8g;薄荷6g),热重加鱼腥草、胆南星;寒重加生姜;痰黏稠加白芥、紫苏子等;咯血丝痰加茜草根、泽兰,水煎500m L,1剂/d,2次/d;西药治疗同对照组。连续治疗15d为1疗程。观测临床症状、FEV_1、FEV_1/FVC、不良反应。治疗1疗程(15d),判定疗效。[结果]治疗组显效26例,有效16例,无效2例,总有效率93.33%;对照组显效21例,有效13例,无效11例,总有效率75.56%;治疗组疗效优于对照组(P<0.05)。FEV_1、FEV_1/FEV两组均有升高(P<0.05),治疗组升高多于对照组(P<0.05)。[结论]养阴清肺汤联合西药治疗老年慢性阻塞性肺气肿(阴虚燥热),疗效满意,无严重不良反应,值得推广。[Objective] To observe the effect of Yangyin Qingfei decoction（养阴清肺汤） combined with western medicine in treating chronic obstructive pulmonary emphysema（Yinxu Zaore/阴虚燥热） in the elderly. [Methods] 90 cases of inpatients were randomly divided into two groups by random parallel control method. In the control group,45 cases of aminophylline 0.1-0.2 g/times,3 times/d; salmeterol and fluticasone,2 times/d,inhalation,levofloxacin 0.5 g/times,1 times/d. In the treatment group,45 cases of Yangyin Qingfei decoction（Baishao,Shengdihuang aeach15 g,Mudanpi,Chuanbei,Maidong,Xuanshen each 10 g,Gancao 8 g,Bohe 6 g）,cold weight plus ginger,phlegm sticky with white mustard and perilla seed,hemoptysis and Rhizoma Rhizoma,zisan,500 m L,1/d,2/d,and 2 times/d; western medicine treated the same Take a photo. The continuous treatment of 15 d was 1 courses. Clinical symptoms,FEV1,FEV1/FVC and adverse reactions were observed. The 1 course of treatment（15 d） was treated and the curative effect was determined. [Results] 26 cases were found in the treatment group,16 cases were effective,2 cases were invalid,the total effective rate was 93.33%,the control group was 21 cases,13 cases were effective,11 cases were invalid and the total effective rate was 75.56%. The curative effect of the treatment group was better than the control group（P〈0.05）. The two groups of FEV1 and FEV1/FEV were increased（P〈0.05）,and the treatment group was higher than that of the control group（P〈0.05）. [Conclusion] Yangyin Qingfei decoction combined with western medicine in the treatment of senile chronic obstructive pulmonary emphysema（Yinxu Zaore） has satisfactory curative effect,no serious adverse reactions,and is worthy of promotion.
