天麻钩藤饮联合硝苯地平治疗高血压(肝阳上亢)随机平行对照研究

摘　　要：[目的]观察天麻钩藤饮联合硝苯地平治疗高血压(肝阳上亢)疗效。[方法]使用随机平行对照方法,将80例门诊患者按就诊顺序号方法随机分为两组。对照组40例硝苯地平,20mg/次,2次/d。治疗组40例天麻钩藤饮(石决明18g,钩藤、牛膝各12g,天麻、山栀、黄芩、杜仲、益母草、桑寄生、夜交藤、茯神各9g),胸闷气短,酌加竹茹、柴胡;大便干结,酌加当归、肉苁蓉;失眠多梦,酌加酸枣仁炒;舌苔厚腻,酌加泽泻、薏苡仁、白术,水煎300m L,1剂/d,早晚分服;西药治疗同对照组。连续治疗6个月为1疗程。观测临床疗效、舒张压、收缩压、不良反应。治疗1疗程(6个月),判定疗效。[结果]治疗组显效30例,有效9例,无效1例,总有效率97.50%;对照组显效20例,有效12例,无效8例,总有效率80.00%;治疗组疗效优于对照组(P<0.05)。收缩压、舒张压两组均有降低(P<0.01),治疗组降低多于对照组(P<0.01)。[结论]天麻钩藤饮联合硝苯地平治疗高血压(肝阳上亢),疗效满意,无严重不良反应,值得推广。[Objective] To observe the curative effect of Tianma Gouteng decoction combined with nifedipine on hypertension（Ganyang Shangkang）. [Methods] Using random parallel control method,80 outpatients were randomly divided into two groups according to the order number method. In the control group,40 cases of nifedipine,20 mg/times,and 2 times/d. In the treatment group,40 cases of Tianma Gouteng decoction（abalone shell 18 g,uncaria,achyranthes bidentata each 12 g,gastrodia elata blume,mountain gardenia,scutellaria,eucommia ulmoides,leonurus,herba taxilli,mulberry parasite,poria with hostwood each 9 g）,chest tightness and shortness of breath,as appropriate bamboo shavings,radix bupleuri;stool dry,as appropriate angelica,cistanche deserticol; insomnia and dreamful sleep,as appropriate semen Ziziphi Spin; thick and greasy fru of the tongue,as appropriate alisma orientalis,coix seed,atractylodes macrocephala,rhizoma atractylodis macrocephala,decoction 300 mL,1 Ji/d,divided intowestern medicine and western medicine. Continuous treatment for 6 months was the 1 course of treatment. Clinical efficacy,diastolic blood pressure,systolic blood pressure and adverse reactions were observed. The 1 course of treatment（6 months） was treated and the curative effect was determined. [Results] 30 cases were found in the treatment group,9 cases were effective,1 cases were invalid,the total effective rate was 97.50%,the control group was 20 cases,12 cases were effective,8 cases were invalid and the total effective rate was 80%. The curative effect of the treatment group was better than the control group（P〈0.05）. Systolic blood pressure and diastolic blood pressure were decreased in two groups（P〈0.01）,and the treatment group was lower than that in the control group（P〈0.01）. [Conclusion] Tianma Gouyengyin combined with nifedipine in treatment of hypertension（Ganyang Shangkang） has satisfactory curative effect,no serious adverse reactions,and is worthy of promotion.
