摘　　要：研究利用信息化互联网技术,构建上级医院与基层医疗机构之间的信息共享交换机制,为基层群众提供优质、高效、便捷、连续的双向转诊医疗服务。分析了上级医院与基层医疗机构的信息化现状,详细介绍了上级医院与基层医疗机构的双向转诊服务模式的设计,包括网络互联架构设计、信息共享机制设计和双向转诊流程设计。最后阐述了上级医院与基层医疗机构之间双向转诊服务模式的实现和取得的成效。The paper studied the application of information internet technology to construct the information sharing and exchange mechanism between the higher hospitals and primary medical institutions that provide high-quality, efficient, convenient and continuous two-way referral medical service for grassroots residents. The paper analyzed the present situation of information in higher hospitals and primary medical institutions and detailed the design of two-way referral service mode in medical association, including the design of network interconnection framework, information sharing mechanism and two-way referral process. Finally, the paper expounded the realization and achievement of two-way referral service model between the higher hospitals and primary medical institutions.
