摘　　要：探讨个案管理模式在高血压患者疾病管理中的应用,本文介绍了高血压个案管理的意义,总结归纳了社区–家庭保健、医院–社区–家庭三元联动和电子信息化3个模式在高血压个案管理中的应用及效果,对高血压个案管理在国内的发展提出展望。To explore the application of case management model in disease management of hypertension patients. This paper introduces the significance of hypertension case management, and summarizes the application and effect of three modes of community-family health care, the hospital-community-household linkage and electronic computerization in hypertension case management. The prospect of the development of hypertension case management in China is also presented.
