腹腔镜完全腹膜外腹股沟疝修补术后并发症的临床分析

摘　　要：目的探讨腹腔镜完全腹膜外腹股沟疝修补术术后并发症的原因及防治措施。方法腹股沟疝病人158例,应用腹腔镜完全腹膜外腹股沟疝修补术治疗,观察病人术后阴囊血肿或浆液肿、尿潴留、缺血性睾丸炎和睾丸萎缩、暂时性神经感觉异常、慢性疼痛、腹股沟区皮下或阴囊气肿等并发症及复发情况。结果手术均获成功,无中转开腹手术,平均手术时间(62.5±10.6)分钟,术后24小时能下床活动,平均住院时间(5.5±1.5)天,全组病人术后随访6~36个月,术后出现并发症13例(8.2%),其中阴囊血肿或浆液肿4例(2.5%),尿潴留3例(1.9%),缺血性睾丸炎和睾丸萎缩1例(0.6%),暂时性神经感觉异常与慢性疼痛3例(1.9%),腹股沟区皮下或阴囊气肿2例(1.3%),随访期间病人未出现复发。结论腹腔镜完全腹膜外腹股沟疝修补术安全有效、疼痛轻、恢复快、并发症少。Objective To investigate the causes and precaution measure of the complications following totally extraperitoneal laparoscopic（ TEP） hernia repair. Methods 158 patients with inguinal hernia were treated with TEP. Postoperative complications（ scrotal hematoma or serous cyst,urinary retention,testicular atrophy and necrosis,transient neurapraxia and chronic pain,inguinal or scrotal emphysema,recurrence rates） were observed at an average follow-up time of 18 months（ ranging form 6 to 36 months）.Results The operations were completed successfully in all of the 158 cases without conversions to open surgery. The operating time was（ 62. 5 ± 10. 6） min. Out of bed after 24 hours. The postoperative hospitalization stay was（ 5. 5 ± 1. 5） days. There were 13 cases（ 8. 2%） suffered complications,involved 4（ 2. 5%）of scrotal hematoma or serous cyst,3（ 1. 9%） of urinary retention,1（ 0. 6%） of testicular atrophy and necrosis,3（ 1. 9%） of transient neurapraxia and chronic pain,2（ 1. 3%） of inguinal or scrotal emphysema.Hernia recurrence have not been found. Conclusion Totally extraperitoneal laparoscopic hernia repair is a safe,simple and effective method with less pain and rapid recovery.
