摘 要:目的探讨血浆血红素加氧酶1(heme oxygenase-1 HO-1)和血浆基质金属蛋白酶9(matrix metalloproteinase-9,MMP-9)与冠心病的相关性。方法选择2011年3月至2014年12月在北京市门头沟区医院心内科疑似冠心病的住院患者292例,其中男性167例,女性125例;年龄34~90(64±12)岁。根据冠脉造影(coronary angiography,CAG)结果分为对照组(143例)和冠心病组(149例),冠心病组根据病变支数又分为单支病变(38例)、双支病变(46例)、多支病变(65例),测量患者HO-1及MMP-9水平,计算冠心病组Gensini积分;同时收集病史、尿酸、血脂、高敏C反应蛋白、糖化血红蛋白等指标,评价HO-1、MMP-9与冠心病的相关性。结果 (1)冠心病组MMP-9及Gensini积分明显高于对照组(P<0.05),HO-1水平明显低于对照组(P<0.05);(2)MMP-9及Gensini积分随着病变血管数增加而升高(P<0.05),HO-I水平则降低(P<0.05);(3)HO-1与Gensini积分呈负相关(r=-0.952,P<0.01),MMP-9与Gensini积分呈正相关(r=0.881,P<0.01),HO-1与MMP-9呈负相关(r=-0.818,P<0.01)。结论血浆中HO-1和MMP-9水平与冠心病有关。metalloproteinase-9 Objective To investigate the correlation of heme oxygenase-1 (HO-1) and matrix (MMP-9) with coronary artery disease (CAD). Methods A prospective cohort study was carried out on 292 inpatients (167 males and 125 females, at a mean age of 64 ± 12 years, ranging from 34 to 90 years) with suspected CAD admitted in our department during March 2011 and December 2014. According to the results of coronary angiography ( CAG), the patients were divided into control group ( n = 143 ) and CAD group ( n = 149). According to the number of vessels involved, the patients of CAD group was further divided into one-vessel subgroup (n = 38), two-vessel subgroup (n = 46), and multi-vessel subgroup ( n = 65 ). The plasma levels of HO-1 and MMP-9 were measured in all the patients, their medical history was collected, and the results of uric acid, blood lipids, high sensitivity C-reactive protein (Hs-CRP), glycosylated hemoglobin were recorded. Gensini' s score was calculated in the CAD group. The correlations of HO-1 and MMP-9 with CAD were analyzed. Results (1) The MPV level and Gensini' s score in the CAD group were obviously higher than those in the control group ( P 〈 0.05 ), while the level of HO-1 was significantly lower (P 〈 0.05 ). (2) With more vessels involved, the MMP-9 level and Gensini' s score were significantly increased ( P 〈 0.05 ), while the HO-1 level was notably decreased ( P 〈 0.05 ). (3) The level of HO-1 was negatively correlated with Gensini' s score ( r = - 0. 952, P 〈 0.01 ), while that of MMP-9 was positively (r =-0. 881, P 〈 0.01 ). Furthermore, the levels of the 2 indicators were negatively correlated (r = -0. 818, P 〈0.01 ). Conclusion Plasma HO-1 and MMP-9 levels are associated with CAD.
