摘 要:目的观察非布司他与别嘌呤醇对痛风患者尿α1微球蛋白(α1-MG)、尿β2微球蛋白(β2-MG)、尿单核细胞趋化蛋白-1(MCP-1)的变化,评估其对肾小管功能的影响及尿MCP-1在早期痛风性肾脏病中的意义方法收集符合标准的样本61例分为三组,非布司它组(非布司他片40mg/d)、别嘌呤醇组(别嘌呤醇片0.3g/d)及对照组。比较初始及治疗后第4周、24周血常规、肝功能、肾功能、尿α1-MG、尿β2-MG、尿MCP-1等指标。结果非布司他组治疗24周后尿α1-MG、尿β2-MG、尿MCP-1水平比治疗前明显下降,与别嘌呤醇组及对照组比较差异均有统计学意义,治疗前后尿液中MCP-1含量与尿α1-MG、β2-MG水平均呈正相关。结论非布司他治疗痛风过程中患者尿液中α1-MG、β2-MG指标均较治疗前下降,提示损伤的肾小管功能好转。尿液MCP-1与尿液α1-MG、β2-MG水平密切相关,尿液MCP-1可能成为痛风患者肾小管功能评估的有效指标。Objective To observe the changes of urinary α1-Microglobulin ( α1-MG ) , urinary β2-microglobulm ( β2-MG ) , and urinary monocyte chemotactic protein-1 (MOP-1) , to evaluate the effect of febuxostat and allopurinol on renal tubule function in patients with gout and the significance of urinary MCP-1 in early gout. Methods The 61 samples that met the standard were divided into three groups, namely febuxostat group: treatment with febuxostat 40mg/d, allopurinol group: treatment with allopurinol 0.3 g/d and the control group. The changes of blood routine, liver function, renal function, urine α1 MG, urine β2-MG and urine MCP-1 were compared. Results The levels of urine α1- MG, urine β2-MG and urine MCP 1 were significantly lower than that in the 24 weeks after the treatment of the febuxostat group, and there was a statistically significant difference between the aUopuriuol group and the control group. The content of MCP-1 in urine before and after treatment was positively correlated with the level of urine α1-MG and β2 MG. Conclusion During the treatment with febuxostat, the patients' urine α1-MG and β2-MG are lower than before treatment, suggests that the damaged renal tubule function is improved. The urine MCP-1 is closely related to urine α1- MG and β2-MG level, and the urine MCP-1 may be an effective indicator for the evaluation of renal tubule function in gout patients.
