针刺治疗肩周炎即刻止痛效果观察

摘　　要：目的:观察循经针刺单穴与针刺肩髃、肩髎、肩贞治疗肩周炎即刻止痛的临床疗效。方法:40例随机分为单穴组和多穴组各20例。单穴组循经取穴,多穴组取肩髃、肩髎、肩贞,两组均只行针刺治疗,比较月经周期治疗前,针刺10min,20min,30min及起针后30min两组疼痛视觉模拟评分(Visual Analogue Scale,VAS)。结果:单穴组与治疗前VAS评分比较,针刺10min、针刺20min、针刺30min和起针后30min VAS评分均明显降低(P<0.05)。多穴组与治疗前VAS比较,针刺20min VAS评分存在差异(P<0.05),针刺30min和起针后30min VAS评分明显降低(P<0.01)。单穴组与多穴组相比,针刺10min、20min、30min和起针后30min VAS评分差异有统计学意义(P<0.05)。治愈率单穴组90%,多穴组65%,两组比较差异有统计学意义(P<0.05)。结论:循经针刺单穴和针刺肩髃、肩髎、肩贞均可明显减轻肩周炎疼痛,且循经针刺单穴治疗肩周炎即刻止痛疗效优于针刺肩髃、肩髎、肩贞。Objective： To observe immediate analgesic effects of acupuncturing single point guided by meridian study and acupuncturing Jianyu point, Jianliao point and Jianzhen point on periarthritis of shoulder patients. Method ： 40 patients were randomly divided into single-point group and multi-point group with 20 patients respectively.The single-point group were treated with single point guided by meridian study while the multi-point group were treated with Jianyu point, Jianliao point and Jianzhen point. The two groups only were treated with acupuncture treatment. The score of Visual Analogue Scale （VAS） of patients in the two group before the treatment, acupuncturing for 10 min, acupuncturing for 20 min, acupuncturing for 30 rain and 30 min after completion of acupuncture was compared and the clinical effects was evaluated. Result： Compared with that before treatment, VAS score of single-point group at the time point of acupuncturing for 10 min, acupuncturing for 20 min, acupuncturing for 30min and 30 min after the completion of treatment significantly decreased （ P〈0.05 ） . Compared with that before treatment, VAS score of multi-points group decreased with acupuncturing for 20 min （ P〈0.05 ） and remarkably decreased with acupuncturing for 30 min and 30 min after the completion of treatment （ P〈0.01 ） ,especially at the time point acupuncturing for 30 min. There was statistical difference between the single-point group and multi-point group in VAS score at the time point of acupuncturing for 10 min, acupuncturing for 20 min, acupuncturing for 30min and 30 min after the completion of treatment （ P〈0.05 ） . The curative rate of the single-point group was 96% while that of the multi-point group was 65% and the total curative rate of the two groups is 100%. Conclusion： Needling single point guided by meridian study or needlding on Jianyu point, Jianliao point and Jianzhen point could both obviously relieve the pain of periarthritis of shoulder.
