头痛宁胶囊联合氟桂利嗪治疗偏头痛有效性及安全性的临床随机对照试验的Meta分析

摘　　要：目的系统评价头痛宁胶囊联合氟桂利嗪治疗偏头痛有效性及安全性的临床效果,为患有偏头痛的患者选择合适的治疗措施提供理论依据。方法一共检索了8个数据库,检索时间范围设定为各数据库建库开始至2017年9月1日。采用软件Review Manager 5.3对入选的文献进行质量评价,Stata12.0进行统计学分析。结果最终纳入28篇文献,Meta分析结果显示:头痛宁胶囊联合氟桂利嗪治疗偏头痛与单独使用氟桂利嗪比较,其总体有效率[RR=1.22,95%CI(1.18,1.26),P<0.001],痊愈率[RR=1.70,95%CI(1.53,1.89),P<0.001]高于对照组;不良反应发生率[RR=0.39,95%CI(0.25,0.62),P<0.001],疼痛程度VAS评分[WMD=-1.10,95%CI(-1.25,-0.95),P<0.001],治疗后头痛发作频数[WMD=-1.86,95%CI(-2.00,-1.71),P<0.001]低于对照组。结论根据本研究结果得到结论为,头痛宁胶囊联合氟桂利嗪治疗偏头痛有效性及安全性高于单独使用氟桂利嗪,两者合用可能取得更好的效果,但尚需更多高质量的研究以增加证据的强度。Objective To choose proper treatment based on systematic review of Toutongning capsule combined with flunarizine concerning clinical effects of efficacy and safety in the treatment of migraine,and to provide theoretical basis for those who suffer from migraine. Methods 8 databases were retrieved in total,and the retrieval time was set from the beginning of all databases to September 1,2017. The quality of the selected literatures was evaluated by Review Manager 5. 3,and Stata12. 0 software was used for statistical analysis. Results 28 literatures were selected in the final and meta-analysis indicates： comparing to use flunarizine alone,Toutongning capsule combined with flunarizine have lead in overall effective rate（ RR = 1. 22,95% CI（ 1. 18,1. 26）,P〈0. 001） and cure rate [RR = 1. 70,95% CI（ 1. 53,1. 89）,P〈0. 001] were higher than that of control group;adverse reaction rate [RR = 0. 39,95% CI（ 0. 25,0. 62）,P〈0. 001],pain degree（ VAS） [WMD =-1. 10,95% CI（-1. 25,-0. 95）,P〈0. 001] and headache attack frequency [WMD =-1. 86,95% CI（-2. 00,-1. 71）,P〈0. 001] were lower than that of control group. Conclusion The study concludes that Toutongning capsule combined with Flunarizine concerning efficacy and safety in the treatment of different types of migraine is superior to using flunarizine alone,but it still needs more studies of high-quality to enhance creditability that Toutongning Capsule combined with Flunarizine could achieve better effect.
