摘 要:目的研究男性雄激素性脱发的遗传性特征。方法收集我院临床363例男性雄激素性脱发患者和30例头皮瘢痕且不伴有脱发患者的临床资料,对其父系家族和母系家族的脱发史进行分析总结,采用Fisher精确检验、Mann-Whitney检验、独立样本t检验进行统计学分析。结果男性雄激素性脱发与遗传密切相关,病例组有家族史的占81.26%,高于对照组的20%(P<0.05)。相同的趋势分别在父/母家族史独立的分析中得到了验证,病例组有父系家族史的患者占64.74%,高于对照组的13.34%,有母系家族史的占30.03%,也高于对照组的6.67%(P<0.05),男性雄激素性脱发主要来源于父系遗传。本研究中有家族史的脱发患者平均发病年龄25岁,无家族史的脱发患者平均发病年龄27岁,两者虽没有统计学差异,但P=0.0582,接近临界值,说明有遗传史(父系或母系)的患者可能比无遗传史的患者发病更早。结论男性雄激素性脱发的患者与父系和母系的遗传都密切相关,在脱发患者中,父系遗传因素相对于母系更易导致脱发的发生。有遗传史(父系或者母系)的患者可能比无遗传史的患者发病更早。Objective to explore the characteristic of male androgenetic alopecia.Methods The clinical data of 363 men with AGA and 30 men with cicatricial alopecia were anlyzed.Results The onset of AGA was closely was related with both the paternal family history and matrilineal family history.The constituent ratio of subjects with family history in case group(81,26%)was higher than in control group(20%)(P0.05).The same trends were replicated by independent paternal/maternal family history datasets.The constituent ratio of subjects with paternal/maternal family history in case group were 64.74% and 30.03% higher than 13.34% and 6.67% in control,respectively.Man with AGA mainly inherited from his father.(P0.05).In this study,the average age of the men with AGA with family history was 25 years old,and the control was 27 years old.There is no statistical difference between the groups,which indicated that patients with family history may have earlier onset than those with no family history.Conclusion The results indicated that the onset of AGA was closely related with the both paternal family history and matrilineal family history.AGA is more likely to occur in the man with paternal family history than matrilineal family history.The age of onset of AGA with family history would probably be earlier than it with no family history.
