高白细胞性白血病患者白细胞分离效率的影响因素分析

摘　　要：目的：探讨影响白细胞分离效率的因素。方法：对2014年2月-2016年12月高白细胞白血病患者（HLL）的36例白细胞分离进行回顾性分析,计算患者的白细胞去除率,应用独立样本t检验和线性相关分析进行白细胞去除率的单因素分析,再用多元线性逐步回归法进行多因素分析。结果：单因素分析显示：白细胞去除率与患者分离前血小板（PLT）计数（r=-0.549,P=0.001）和中性粒细胞（NEU）计数（r=-0.725,P=0.001）负相关,与分离前红细胞压积（HCT）（r=0.524,P=0.001）正相关。多因素分析显示：分离前NEU（P=0.016）和HCT（P=0.043）是白细胞去除率的影响因素。结论：分离前NEU和HCT是影响白细胞去除率大小的因素。分离前NEU值较低,HCT值较高的患者,更容易获得较好的分离效果。Objective：To investigate factors influencing the efficiency of leukocytapheresis（EFF_（WBC））for patients with hyperleukocytic leukemia（HLL）.Methods：A total of 36 cases of HLL who underwent leukocytapheresis were retrospectively analyzed between Feb.2014 and Dec.2016.Twosample T test and linear correlation analysis were applied to explore factors related to EFF_（WBC）.Multiple linear regression with stepwise variable selection was then performed to indicate factors influencing EFF_（WBC）.Results：Univariable analysis showed that there was a significant variation in statistics including PLT（r=-0.549,P=0.001）,NEU（r=-0.725,P=0.001）,and HCT（r=0.524,P=0.001）for EFF_（WBC） before leukocytapheresis.Multivariate analysis indicated that only NEU and HCT before leukocytapheresis had a significant influence on EFF_（WBC）.Conclusion：NEU（P=0.016）and HCT（P =0.043）before leukocytapheresis are the influencing factors of EFF_（WBC）.HLL patients with lower NEU counts and higher HCT are expected to have higher EFF_（WBC）.
