2014-2016年江苏省张家港市县、乡两级医院消毒效果监测结果分析

摘　　要：目的了解张家港市2014-2016年医疗机构消毒状况,保证消毒效果,防止医源性感染的发生。方法对张家港市医疗机构的室内空气、物体表面、医护人员手、使用中消毒液、灭菌器械、压力蒸汽灭菌、胃肠镜、紫外线灯和医院污水进行采样和检测。结果 2014-2016年共采样2 372份,总合格率为93.25%,各年消毒质量合格率分别为91.98%、92.73%和95.03%,差异有统计学意义(χ~2=6.47,P<0.05)。各监测项目合格率从高至低分别为:使用中消毒液100.00%、物体表面98.98%、灭菌器械98.25%、室内空气97.40%、压力蒸汽灭菌93.75%、紫外线灯90.85%、医护人员手89.33%、胃肠镜88.00%和医院污水55.14%,差异有统计学意义(χ~2=326.73,P<0.05)。不同类型医疗机构消毒质量合格率分别为县级医院94.43%、乡镇医院93.02%。结论张家港市医疗机构消毒质量合格率呈逐年上升趋势,但医院污水的合格率较低,应加强监督和管理。Objective To investigate the disinfection status of medical institutions in Zhangjiagang during 2014-2016,and to ensure the disinfection effect and prevent the occurrence of iatrogenic infection. Methods To detect the microorganism of indoor air,surfaces of objects,hands of medical staff,the disinfection in use,sterilization equipment,pressure steam sterilization,gastrointestinal endoscopy,ultraviolet lamp and hospital sewage in medical institutions of the city. Results A total of 2 372 samples were collected in the past three years,the total pass rate being 93. 25%. The qualified rate of disinfection quality during2014-2016 was 91. 98%,92. 73% and 95. 03%,respectively. The difference was statistically significant（ χ^2= 6. 47,P〈0. 05）. The qualified rate of each monitoring project from high to low was disinfectants in use（ 100. 00%）,surface of subjects（ 98. 98%）,sterilized instruments（ 98. 25%）,air in hospital（ 97. 40%）,indoor air pressure steam sterilization（ 93. 75%）,ultraviolet lamp（ 90. 85%）,hands of medical staff（ 89. 33%）,gastrointestinal endoscopy（ 88. 00%）,sewage in hospital（ 55. 14%）,respectively（ χ^2= 326. 73,P〈0. 05）. The qualified rate of disinfection quality of different types of medical institutions of county hospitals was 94. 43%,and township hospitals was 93. 02%. Conclusion The qualified rate of disinfection quality of medical institutions in Zhangjiagang is generally increasing year by year, but the qualified rate of hospital sewage is low. Supervision and management should be strengthened.
