补中益气汤联合盐酸米多君治疗帕金森病直立性低血压疗效研究

摘　　要：目的:观察补中益气汤联合小剂量盐酸米多君治疗帕金森病(PD)的直立性低血压(OH)疗效。方法:对照组单用推荐剂量盐酸米多君治疗,联合治疗组为中药方剂联合小剂量盐酸米多君治疗,依据中医辨证分型采用补阳还五汤加减,分别观察两组治疗每周的血压变化,3周后的总疗效,药物不良反应发生率以及血压对两组平衡功能的影响。结果:方剂联合治疗组治疗第三周血压较治疗前明显升高,卧位(138.11±13.03/90.35±7.04)mmHg、立位(125.21±13.11/70.04±4.07)mmHg vs卧位(90.33±12.14/70.58±10.03)mmHg、立位(70.26±13.16/55.43±8.12)mmHg),对照组治疗第一周血压较治疗前明显升高,卧位(125.13±15.10/85.19±9.03)mmHg、立位(116.29±14.01/80.17±11.35)mmHg与卧位(95.22±9.08/76.33±11.05)mmHg、立位(73.42±11.07/62.14±10.17)mmHg,统计学均有差异(P<0.05)。比较联合治疗组第三周与对照组第一周的血压无统计学差异,两组3周后升压总疗效比较也无统计学差异。联合治疗组治疗药物不良反应发生率明显低于对照组有统计学差异(P<0.05)。两组治疗3周后较治疗前的平衡功能均有改善,治疗3周后的Berg量表评分明显提高,有统计学差异,两组间比较无统计学差异。结论:补中益气汤联合小剂量盐酸米多君治疗帕金森患者OH的临床疗效没有降低,与单用常规剂量盐酸米多君疗效相同,两组OH的有效治疗均能明显改善患者的平衡功能。补中益气汤联合盐酸米多君治疗能明显减少盐酸米多君的不良反应。Objective：Study on traditional Chinese medicine Buzhong Yiqi decoction combined with midodrine hydrochloride in treatment of orthostatic hypotension（OH）in Parkinsons disease（PD）.Methods：The control group was received routine treatment with midodrine hydrochloride while the experimental group was received Buzhong Yiqi decoction and midodrine hydrochloride.observation of blood pressure changing every week,total curative ratio three weeks after treatment and occurrence rate of midodrine hydrochloride side effect were performed.The effect of blood pressure on blance function was evaluated with berg blance scale.Results：Blood pressure of the experimental group elevated significantly three weeks after treatment,supine position（138.11±13.03/90.35±7.04）mmHg,orthostatic position（125.21±13.11/70.04±4.07）mmHg vs supine position（90.33±12.14/70.58±10.03）mmHg,orthostatic position（70.26±13.16/55.43±8.12）mmHg as well as the contol group the first week after treatment,supine position（125.13±15.10/85.19±9.03）mmHg,orthostatic position（116.29±14.01/80.17±11.35）mmHg vs supine position（95.22±9.08/76.33±11.05）mmHg,orthostatic position（73.42±11.07/62.14±10.07）mmHg.Experimental group and control group dividedly showed significant difference in blood pressure three weeks after treatment compared to before treatment,but no significant difference between the two goups.There was no significant deference in total curative ratio between the two groups three weeks after treatment.The occurrence rate of midodrine hydrochloride side effect in experimental group three weeks after treatment was significantly lower compared to control group.The blance function of two groups were improved and the scores of BBS were significantly elevated,but no significant difference between the two goups.Conclusion：Buzhong Yiqi decoction combined with half dose midodrine hydrochloride in treatment of OH in PD showed the same therapeutic effect as routine dose midodrine hydrochloride used alone.The effec
