床边防跌倒护理路径表在肾内科的应用

摘　　要：目的探讨床边悬挂防跌倒护理路径表(下称路径表)在预防肾内科患者跌倒中的应用效果。方法选取肾内科有跌倒风险(评分≥2分)的住院患者1114例,其中2014年6月1日至2015年12月31日收治的598例患者(对照组);2016年1月1日至2017年5月31日收治的515例(观察组),2组按同一路径表的风险要求对患者进行评估、宣教并落实预防措施。对照组将路径表保存于电脑护理记录系统中,观察组将路径表悬挂于患者床旁并请患者、家属或陪护签名,比较2组的跌倒发生率。结果观察组、对照组发生跌倒分别为2例和9例,其中对照组3例跌倒发生于晨起,6例无陪护时发生跌倒;观察组2例跌倒均发生于无陪护。观察组跌倒发生率低于对照组(1.75%比0.33%,χ2=4.295、P<0.05)。结论实施床边悬挂路径表便于责任组长/护士长的督导,有助于责任护士对患者采取个性化的防跌倒措施,可降低住院患者跌倒发生率。Objective To explore the efficacy of bedside hanging fall prevention nursing path table in fall prevention in the department of nephrology.Methods A total of 1114 patients with fall risk score≥22 were divided into control group（598 patients treated between June 1,2014 and December 31,2015）and observation group（515 patients treated between January 1,2016 and May31,2017）.The assessment,education and prevention of fall risk were performed according to the same path table requirements.In control group,the nursing path table was stored in computer nursing recording system.In observation group,the nursing path table was hanged beside the bed and patients,family members or escorts were required to sign their signature.The incidence of fall was compared between the two groups.Results In observation group,the fall occurred in 2 patients without escorts.In control group,3 patients fell in the morning and 6 patients fell in the absence of escorts.The incidence of fall in observation group was lower than that in control group（1.75% vs 0.33%,χ2=4.295、P〈0.05）.Conclusion The application of bedside hanging fall prevention nursing path table facilitates the supervision of team leader or head nurse,contributes to performance of personalized preventive measures and decreases the incidence of fall in inpatients.
