陈氏肾9方治疗特发性膜性肾病的临床观察

摘　　要：目的观察陈氏肾9方治疗特发性膜性肾病的临床疗效。方法将60例特发性膜性肾病患者随机分为治疗组和对照组,每组30例。对照组予糖皮质激素+环磷酰胺治疗,治疗组予陈氏肾9方治疗。两组疗程均为12个月,观察临床疗效及用药安全性,比较24 h尿蛋白定量、血浆白蛋白(Alb)、血肌酐(Scr)的变化情况。结果 (1)治疗组、对照组总缓解率分别为76.7%、59.3%;组间临床疗效比较,治疗组明显优于对照组(P<0.05)。(2)治疗前后,两组24 h尿蛋白定量、Alb水平差异均有统计学意义(P<0.05),而Scr水平差异均无统计学意义(P>0.05)。组间治疗后比较,治疗组24 h尿蛋白定量水平明显低于对照组(P<0.05),而治疗组Alb水平明显高于对照组(P<0.05)。(3)试验期间,两组不良反应发生率比较,差异有统计学意义,治疗组明显低于对照组(P<0.05)。结论陈氏肾9方治疗特发性膜性肾病的疗效满意且不良反应较少,尤其适用于常规西药治疗不敏感的患者。Objective To observe the clinical efficacy of ＂Chen＇s No.9 Formula for kidney diseases＂in the treatment of idiopathic membranous nephropathy.Methods 60 patients with idiopathic membranous nephropathy were randomly divided into the treatment group and control group,30 cases in each group.The control group was treated with glucocorticoid plus cyclophosphamide and the treatment group was treated with＂Chen＇s No.9 Formula for kidney diseases＂,with a course of 12 months.The clinical efficacy and the medication safety were observed.The changes on 24 h urine protein quantity,plasma albumin（Alb） and serum creatinine（Scr） were compared.Results（1）The total remission rates were 76.7% in the treatment group and 59.3% in the control group.The clinical efficacy of the treatment group was obviously better than that of the control group（P〈0.05）.（2）In both groups,there were statistically significant differences on the levels of24 h urine protein quantity and Alb between treatment before and after（P〈0.05）,while there was no statistically significant difference on the Scr level between treatment before and after（P〈0.05）.After treatment,the level of 24 h urine protein quantity in the treatment group was obviously lower than that in the control group（P〈0.05）,while the Alb level in the treatment group was obviously higher than that in the control group（P〈0.05）.（3）During the trial,there was statistically significant difference on the incidence rate of adverse reactions between the two groups,the incidence rate of adverse reactions in the treatment group was obviously lower than that in the control group（P〈0.05）.Conclusion ＂Chen＇s No.9 Formula for kidney diseases＂shows satisfactory efficacy in the treatment of idiopathic membranous nephropathy with fewer adverse reactions,especially for the patients who are not sensitive to routine western medicine.
