超声多普勒引导下痔动脉结扎加悬吊术治疗30例老年Ⅱ～Ⅲ度混合痔疗效分析

摘　　要：目的分析超声多普勒引导下痔动脉结扎加悬吊术治疗老年Ⅱ~Ⅲ度混合痔的临床疗效。方法以2013年1月至f2016年12月于南京市中医院肛肠科住院治疗的30例老年Ⅱ~Ⅲ度混合痔患者为例,分析行超声多普勒痔引导下动脉结扎加悬吊手术的治疗效果,记录治愈率、住院时间、术后疼痛、术后并发症及随访情况。结果 30例患者平均住院时间为(7.50±0.51)d;术后3例患者发生尿潴留、导尿后好转,术后疼痛评分均在0~2分、未见需对症处理的病例,2例患者发生肛缘水肿、症状自行好转。术后1个月时,30例患者治愈率为100%。术后随访6~24个月,无复发,无肛门狭窄等并发症发生。结论超声多普勒引导下痔动脉结扎加悬吊术操作简便,并发症较少,疗效良好,适用于老年Ⅱ~Ⅲ度混合痔的治疗。Objective To study the clinical efficacy of Doppler-guided hemorrhoid artery ligation and suspension in the treatment of older patients with mixed hemorrhoids of Ⅱ to Ⅲ degree. Methods 30 older patients with mixed hemorrhoids of Ⅱ to Ⅲ degree were recruited in Department of Anorectal Diseases, Nanjing Hospital of Traditional Chinese Medicine, from January 2013 to December 2016. The effect of Doppler-guided hemorrhoid artery ligation and suspension operation was analyzed. Cure rate, duration of hospitalization, postoperative pain, and postoperative complications were recorded. Results Average hospitalization duration was（7.50±0.51）d. After operation, 3 patients had urinary retention and was successfully dealt with by catheterization. Postoperative pain scores ranged from 0 to 2 points and no case needed active treatment. 2 cases had swelling and symptom subsided without treatment. At 1 month, the cure rate was 100%. At 6-24 months follow-up, there was no recurrence, anal stenosis or other complications. Conclusion Dopplerguided hemorrhoid artery ligation and suspension was simple and easy to operate with less complications and good curative effect. It was suitable for the treatment of older patients with mixed hemorrhoids of Ⅱ to Ⅲ degree.
