寒区军人功能性便秘调查分析

摘　　要：目的了解寒区官兵功能性便秘的患病情况,并分析患病官兵的心理状况。方法选取东北地区北纬45°~50°(该地区年平均温度0℃左右)驻军官兵480名为研究对象。应用成人功能性胃肠病罗马Ⅳ诊断性问卷对寒区480名官兵进行功能性便秘发病情况的问卷调查,并设计成人功能性胃肠病相关心理问题调查问卷对患病官兵进行心理状况调查。记录官兵功能性便秘的发病率,并比较不同兵龄官兵功能性便秘的发病率、功能性便秘官兵及无功能性便秘官兵的心理问卷评分,以及不同程度功能性便秘官兵的心理问卷评分。结果本研究共收集462份有效问卷,功能性便秘官兵33例,发病率为7.1%(33/462);新兵功能性便秘发病率最高,为9.5%(12/126)。功能性便秘组官兵心理问卷评分为(12.3±5.8)分,显著高于无功能性便秘组的(3.8±4.6)分,组间比较,差异有统计学意义(P<0.05)。功能性便秘组官兵中,重度症状者心理问卷评分为(13.7±6.5)分,显著高于轻、中度症状者的(5.1±3.53)分、(8.2±5.1)分,组间比较,差异有统计学意义(P<0.05)。结论根据罗马Ⅳ标准,寒区官兵功能性便秘发病率仍较高,且影响官兵的心理状况,需要进一步完善现有针对寒区官兵功能性便秘的卫勤保障措施。Objective To investigate the incidence of functional constipation（ FC） and psychological state among the soldiers under severe cold condition and contribute to the precention of medical support. Methods Based on the standard questionnaire of Rome IV,a questionnaire survey of FC incidence and psychological state were carried out among 480 soldiers under severe cold condition（ northeast north latitude 45 °-50 °（ annual average temperature in the region around 0℃）. The incidence of FC was recorded and the incidence of FC between different time of being soldiers,soldiers with FC and the ones without FC were accessed by psychological questionnaire score,and the psychological questionnaire scores of different degree of FC were compared. Results A total of 462 valid questionnaires were collected in this study,33 cases of FC,the incidence was 7. 1%（ 33/462）. The highest incidence of FC was 9. 5%（ 12/126）. The score of psychological questionnaire for soldiers in the FC group was（ 12. 3 ± 5. 8）,which was significantly higher than that of the non-FC group（ 3. 8 ± 4. 6）,and the difference was statistically significant（ P〈 0. 05）. The psychological questionnaire score[（ 13. 7 ± 6. 5） scores] was higher than that in the soldiers with mild symptom[（ 5. 1 ± 3. 53） scores]and the moderate symptom[（ 8. 2 ± 5. 1） scores],（ P〈 0. 05）. Conclusion The incidence of FC is higher among the soldiers under severe cold condition based on the standard of Rome IV,and it affects the psychological state. Therefore,the existing measures of medical support under severe cold condition to prevent and treatment FC should to be improved.
