摘　　要：目的探讨专科与家庭责任医生签约模式结合对控制老年高血压的效果。方法选取我院社区中心2016年1~12月的老年高血压患者80例,分成对照组和研究组,分别给予常规社区干预措施和实施专科与家庭责任医生签约模式结合的干预方式。对两组患者干预6个月后的治疗依从情况和血压情况进行观察和比较。结果 (1)干预前,两组患者的治疗依从性评分水平均不高,差异无统计学意义;干预后两组患者的治疗依从性评分均得到了显著改善,与干预前比较差异有统计学意义,且研究组患者的治疗依从性评分显著高于对照组;(2)干预后研究组患者的收缩压和舒张压均明显低于对照组。结论专科与家庭责任医生签约模式结合的健康管理能够有效的改善老年高血压患者的治疗依从情况,有效控制患者血压水平。Objective To investigate the effect of the combination of specialist and family responsibility doctors on the control of hypertension in the elderly. Methods Eighty elderly hypertensive patients in our community center in January 2016 to December 2016 were selected and divided into control group and research group.They were given conventional community intervention measures and the implementation of the professional and family doctor contract mode of intervention.The compliance and blood pressure of the two groups of patients after 6 months of intervention were observed and compared. Results（1） Before the intervention,patients of two groups with treatment compliance score levels were not high.There were no significant difference.After the intervention,the treatment compliance score of the two groups were significantly improved.There were significant differences in comparison.The treatment compliance and scores of patients in the study group was significantly higher than the control group;（2）The intervention of study group patients with systolic and diastolic blood pressure were significantly lower than the control group. Conclusion The health management combined with the contract mode of family responsibility doctors can effectively improve the treatment compliance of elderly hypertensive patients,and effectively control the blood pressure level of patients.
