早期肠内营养对改善重症颅脑损伤患者炎症反应的临床效果分析

摘　　要：目的分析早期肠内营养(EEN)对改善重症颅脑损伤患者炎症反应的临床效果。方法收集我院自2014年8月~2017年9月收治的重症颅脑损伤患者80例作为观察对象,按随机数字表法分为观察组与对照组,每组各40例。对照组采取肠外营养治疗,观察组则采取早期肠内营养治疗。分别于干预后1、3、7、14d测定两组血清C反应蛋白(CRP)水平变化情况。结果干预后观察组ALB(31.57±2.53)cm、TP(69.22±3.57)g/L、BMI(18.32±1.73)kg/m^2、TSF(6.72±1.55)mm、HB(126.44±5.65)g/L均显著优于对照组,两组比较差异具有统计学意义(P<0.05);治疗后第1d时,两组CRP水平比较差异无统计学意义(P>0.05);而在治疗后3、7、14d时,观察组CRP水平均分别显著低于对照组(P<0.05)。结论早期肠内营养有助于改善重症颅脑损伤患者的机体内环境,减轻炎症反应,促进恢复,具有较高的推广价值。Objective To analyze the clinical effect of early enteral nutrition（EEN） on improving inflammatory response in patients with severe craniocerebral injury. Methods Eighty cases with severe craniocerebral injury in our hospital from August 2014 to September 2017 were collected and randomly divided into two groups（observation group and control group）,40 cases in each group.The control group was treated with parenteral nutrition,while the observation group was treated with early enteral nutrition.The level of serum C-reactive protein（CRP） was measured on day 14 after intervention. Results After intervention,ALB（31.57±2.53）cm, TP（69.22±3.57）g/L,BMI（18.32±1.73）kg/m^2,TSF（6.72±1.55）mm,HB（126.44±5.65）g/L of the observation group were significantly higher than the control group,the difference between the two groups was statistically significant（P〈0.05）.At the 1 st day after treatment, there was no significant difference in CRP level between the two groups（P〉0.05）.The levels of CRP in the observation group were significantly lower than those in the control group（P〈0.05） at 3,7,14 d after treatment. Conclusion Early enteral nutrition is helpful to improve the environment of patients with severe craniocerebral injury,alleviate inflammatory reaction and promote recovery,and has high popularization value.
