血小板参数与输注疗效及预后判断的相关性分析

摘　　要：目的探析血小板参数与输注疗效及预后判断的相关性。方法以2015年10月~2017年10月在我院输注单采血小板350例患者为研究对象,分为A组(n=200)和B组(n=150),分别测定两组患者的各项血小板参数,并对血小板参数与输注疗法和预后的相关性进行回顾性分析。结果 (1)A组中24h血小板纠正计数指数>4.5的所占比例高于B组,差异有统计学意义(P<0.05)。(2)输注后,A组患者的血小板计数、血小板分布宽度、平均血小板体积、大血小板比率均明显高于B组,差异有统计学意义(P<0.05)。(3)A组患者输血前后的平均血小板体积、大血小板比率均升高,B组患者的平均血小板体积、大血小板比率均下降。结论平均血小板体积和大血小板比率的变化趋势与血小板输注的治疗效果和预后判断有显著相关性,可以作为临床判断的参考指标。Objective To explore the correlation between platelet parameters and the efficacy and prognosis of infusion. Methods Three hundred and fifty patients with single platelet transfusion in our hospital were divided into group A and group B.The platelet parameters of the two groups were measured,and the correlation between platelet parameters and transfusion therapy and prognosis was analyzed retrospectively. Results（1）The percentage of 24 h platelet correction index 〉4.5 in group A was significantly higher than that in group B（P〈0.05）.（2）the platelet count,platelet distribution width, mean platelet volume and large platelet ratio in group A were significantly higher than those in group B（P〈0.05）.（3）The average platelet volume and large platelet ratio of patients in group A before and after blood transfusion increased,and the average platelet volume and platelet ratio in group B decreased. Conclusion The change trend of mean platelet volume and large platelet ratio was significantly correlated with the therapeutic effect and prognosis of platelet transfusion, and could be used as a reference index for clinical evaluation.
