摘 要:目的探究血清PLA_2水平测定应用于尿毒症患者中的意义。方法选取2016年6月~2017年12月期间我院收治的70例尿毒症患者及健康体检者60例作为研究对象,检测全部入选研究对象磷脂酶A2(PLA_2)水平,将70例尿毒症患者作为病例组,60例健康体检者作为对照组,同时对比透析患者与非透析患者的血清PLA_2水平,而且观察血清PLA_2水平与营养状况、病死率等指标的相关性。结果非透析组PLA_2活性(175.69±30.57)U明显高于透析组及对照组,对比具有显著差异(P<0.05);透析组PLA_2活性(128.96±25.85)U明显高于对照组,对比具有显著差异(P<0.05)。PLA_2活性与血磷水平、血钙磷乘积均具有明显相关性(P<0.05),与血钙指标无明显相关性(P>0.05)。生存者的PLA_2活性为(72.36±22.36)U明显低于透析前(146.96±25.39)U,对比具有显著差异(P<0.05)。对营养状况不良、性别等数据调整,其病死率比高PLA_2活性患者增加(RR=1.8,95%CI为0.8~3.8,P<0.05)。结论血清PLA_2水平测定应用于尿毒症患者中,能够预测其病情及预后效果,具有较高的应用价值。Objective To explore the significance of serum PLA2 level in patients with uremia. Methods 70 cases of uremic patients and 60 healthy volunteers in our hospital were selected as the research object from June 2016 to December 2017.The level of phospholipase A2(PLA2) of all selected subjects was detected. 70 cases of uremic patients were selected as case group,and 60 healthy volunteers were selected as control group.At the same time, the level of serum PLA2 in patients with dialysis and non dialysis patients was compared. The correlation between serum PLA2 level and nutritional status, mortality and other indicators was observed. Results The activity of PLA2 in the non dialysis group[(175.69 ± 30.57) U] was significantly higher than that of the dialysis group and the control group, with significant difference(P〈0.05).The activity of PLA2 in the dialysis group[(128.96±25.85)U] was significantly higher than that in the control group, with significant difference(P〈0.05).The activity of PLA2 was significantly correlated with the level of blood phosphorus and the product of blood calcium and blood phosphorus(P〈0.05), and had no significant correlation with the blood calcium index(P〉0.05).The activity of PLA2 in survivors(72.36±22.36) U, was significantly lower than that before dialysis(146.96±25.39) U, with a significant difference(P〈0.05).The mortality rate was higher than that of PLA2 highly active patients(RR=1.8, 95%CI 0.8-3.8, P〈0.05). Conclusion Serum PLA2 level is used to predict the prognosis and prognosis of uremia patients. It has high application value.
