小剂量肝素预防小儿紫癜性肾炎的临床研究

摘　　要：目的探讨小剂量肝素预防小儿紫癜性肾炎的临床研究。方法将我院收治的42例过敏性紫癜患者随机分为两组,各21例。对照组给予常规抗紫癜治疗,试验组在对照组基础加用小剂量肝素。比较两组患者临床疗效、紫癜性肾炎发生率的差异,并分析其血液流变学及IL-6及TNF-α水平情况。结果试验组临床总有效率为90.5%,高于对照组61.9%(P<0.05);试验组患者紫癜性肾炎发生率为4.8%,低于对照组33.3%(P<0.05);治疗后试验组患者全血黏度、血浆黏度、红细胞比容显著降低,也低于同期对照组(P<0.05);治疗后试验组患者IL-6、TNF-α水平降低,也低于同期对照组(P<0.05)。结论小剂量肝素预防小儿紫癜性肾炎患者效果佳,可降低相关血液流变学及炎症介质,值得临床选择。Objective To investigate the clinical effect of low dose heparin on children with henoch-schonlein purpura nephritis. Methods Forty-two patients with henoch-schonlein purpura admitted to our hospital were randomly divided into two groups,21 cases in each group.The control group was given routine anti-purpura treatment,while the experimental group was given low-dose heparin on the basis of the control group.The clinical efficacy and incidence of henoch-schonlein purpura nephritis between the two groups were compared,and their hemorheology,IL-6 and TNF-α levels were analyzed. Results The total effective rate of the experimental group was 90.5%,which was higher than that of the control group 61.9%（P〈0.05）.The incidence of Henoch-Schonlein purpura nephritis in the experimental group was 4.8%,lower than that in the control group（33.3%）（P〈0.05）.After treatment,the whole blood viscosity, plasma viscosity and red blood cell volume of the patients in the experimental group were significantly lower than those in the control group at the same period（P〈0.05）.After treatment,the levels of IL-6 and TNF-α in the experimental group were lower than those in the control group（P〈0.05）. Conclusion Low dose heparin has good effect in preventing children with henoch-schonlein purpura nephritis,it can reduce related hemorheology and inflammatory mediators,and is worthy of clinical selection.
