腹腔镜卵巢囊肿剥除术超声刀和缝合止血两种止血方法对卵巢功能的影响

摘　　要：目的探究腹腔镜卵巢囊肿剥除术超声刀和缝合止血两种止血方法对卵巢功能的影响。方法回顾性分析我院给予超声刀止血(观察组)和给予缝合止血(对照组)的择期腹腔镜卵巢囊肿剥除术患者各42例临床资料。记录两组手术情况(手术时间、术中出血量、术后住院时间),并比较两组术前(T0)、术后3个月(T1)、术后6个月(T2)时卵巢储备功能[促卵泡刺激素(FSH)、抗苗勒管激素(AMH)、窦状卵泡计数(AFC)、卵巢基质动脉血流收缩期峰值(PSV)]差异。结果观察组手术时间、术中出血量及术后住院时间均低于对照组(P<0.05)。T0及T2时,两组组内和组间比较,差异均无统计学意义(P>0.05);T1时,两组血清FSH水平均组内T1比T2时升高(P<0.05),AMH水平及AFC、PSV检测结果均较组内T1比T2时降低(P<0.05),且观察组变化幅度大于对照组(P<0.05)。结论超声刀和缝合止血两种止血方法均能保护腹腔镜卵巢囊肿剥除术远期卵巢储备功能,但超声刀止血法止血效果优于缝合止血法,于患者预后更有利。Objective To explore the influence of ultrasonic scalpel and suture hemostasis on ovarian function in laparoscopic ovarian cystectomy. Methods Retrospective analysis was made on 42 patients with laparoscopic ovarian cystectomy who were given ultrasonic scalpel hemostasis（observation group） and suture hemostasis（control group） in our hospital.The operation condition（operation time,intraoperative blood loss and postoperative hospital stay） of the two groups were recorded.The differences in ovarian reserve function [promoting follicle stimulating hormone（FSH）,anti Mullerian hormone（AMH）,atrial fibrillation count（AFC） and peak systolic velocity of ovarian stromal artery（PSV） were compared between two groups before operation（T0）,3 months after operation（T1） and 6 months after operation（T2）. Results The operative time,intraoperative blood loss and postoperative hospital stay in the observation group were lower than those in the control group（P〈0.05）.At T0 and T2,the differences within-groups and betweengroups were not statistically significant（P〈0.05）.At T1,the serum FSH levels in both groups were higher than those at T2（P〈0.05）.AMH level and AFC and PSV detection results were lower than those of T1 and T2 in the group（P〈0.05）.And the change range of observation group was larger than that of control group（P〈0.05）. Conclusion Both ultrasonic scalpel hemostasis and suture hemostasis can protect the long-term ovarian reserve function of laparoscopic ovarian cystectomy,but the hemostatic effect of ultrasonic scalpel is superior to suture hemostasis,which is more favorable for the prognosis of patients.
