应用MIPPO技术三种内固定治疗胫骨中下段骨折的比较研究

摘　　要：目的探讨应用MIPPO技术LCP固定、传统普通LC-DCP固定及交锁髓内钉固定治疗胫骨中下段骨折的疗效对比。方法回顾性分析2013年9月~2017年9月于我院就诊并行手术治疗的胫骨中下段骨折患者150例为研究对象。按照其内固定方式不同分为三组,其中A组(50例)实施经皮微创技术锁定加压钢板固定手术(MIPPO-LCP组),B组(50例)实施交锁髓内钉固定手术(髓内钉组),C组(50例)实施有限接触动力加压接骨板固定手术(LC-DCP组)。比较三组患者手术及术后相关情况并做统计分析。结果 MIPPO-LCP组患者治愈率为94.00%,髓内钉治疗组及LC-DCP组治愈率均为92.00%,三组比较差异无统计学意义(P>0.05)。三组患者手术时间比较差异无统计学意义(P>0.05)。术中出血量相比,MIPPO-LCP组显著少于其他两组患者(P<0.05)。MIPPO-LCP组骨折愈合时间显著短于其他两组(P<0.05)。结论应用MIPPO技术LCP内固定治疗闭合性胫骨中下段骨折具有较高的治愈率,创伤小,愈合快,出血少等优点,值得临床推广应用。Objective To evaluate the effect of LCP fixation with MIPPO technique,conventional LC-DCP fixation and interlocking intramedullary nail fixation in the treatment of middle and lower tibial fractures. MethodsFrom September 2013 to September 2017,150 patients with middle and lower tibia fractures treated in our hospital were retrospectively analyzed.They were divided into three groups according to their internal fixation,of which group A（50 cases） carried out percutaneous minimally invasive locking compression plate fixation（group MIPPO-LCP）,group B（50 cases） performed interlocking intramedullary nail fixation（intramedullary nail group）,and group C（50 cases） implemented limited contact dynamic compression plate fixation（group LC-DCP）.The operation and postoperative phase of the three groups were compared.The operative and postoperative complications of three groups were compared and the statistical analysis was performed. Results The cure rate of MIPPO-LCP group was 94.00% and that of intramedullary nail treatment group and LC-DCP group was 92%,there was no significant difference among the three groups（P〈0.05）.There was no significant difference in operative time among the three groups（P〈0.05）.The amount of intraoperative bleeding in MIPPO-LCP group was significantly shorter than that in the other two groups（P〈0.05）.The fracture healing time in MIPPO-LCP group was significantly shorter than that in other two groups（P〈0.05）. Conclusion The application of MIPPO technique LCP internal fixation in the treatment of closed middle and lower tibial fractures has the advantages of high cure rate,small trauma,quick healing and less bleeding,etc.It is worth popularizing and applying in clinic.
