雷火灸用于脑卒中后上肢痉挛的效果观察

摘　　要：目的探讨雷火灸用于脑卒中后患侧上肢痉挛的效果，为脑卒中后患侧上肢痉挛的康复治疗提供依据。方法选择北京中医药大学东直门医院东区2015年8月至2016年8月收治的脑卒中后患侧上肢痉挛患者120例为观察对象，采用随机数字表法分为雷火灸治疗组（观察组）和常规治疗组（对照组），每组60例。对照组采用传统的常规治疗与护理，观察组在对照组的基础上对患者进行雷火灸治疗与护理。观察两组上肢功能恢复情况、临床疗效及患者满意度。结果治疗后观察组的痉挛程度评分为（19．7±2．1）分，明显优于对照组的（26．8±2．6）分，差异有统计学意义（t=4．261，P=0．023）。观察组有效率为94．0％（47／50），明显高于对照组的75．9％（37／50），差异有统计学意义（χ^2=4．346，P=0．021）。观察组患者的总满意率为95．0％（57／60），显著高于对照组的76．7％（40／60），差异有统计学意义（χ^2=4．016，P=0．028）。结论雷火灸可以有效地缓解脑卒中后患侧上肢痉挛，降低Ashwoah痉挛量表痉挛程度评分、临床疗效及患者满意度。Objective To investigate the effect of thunderstorm moxibustion in the treatment of upper extremity spasm after stroke, and to provide basis for the rehabilitation nursing of the upper limb spasticity after clinical stroke. Methods From August 2015 to August 2016,A total of 120 patients with upper limb spasticity after stroke in Dongzhimen Hospital were selected as study objects and divided into two groups according to the random number table,with 60 cases in each group. The observation group received thunder fire moxibustion treatment on the basis of conventional treatment. The control group was treated by traditional routine treatment and nursing. The recovery of upper limb function, clinical efficacy and patients＇ satisfaction were observed. Results After treatment, the degree of spasm in the observation group was （ 19.7 ± 2.1 ） points, which was significantly better than （ 26.8± 2.6 ） points in the control group,the difference was statistically significant（ t =4.261 ,P =0.023）. The effective rate of the observation group was 94.0% （47/50） ,which was significantly higher than 75.9% （37/50） of the control group, the difference was statistically significant（χ^2 =4. 346,P =0. 021 ）. The total satisfaction rate of the observation group was 95.0% （ 57/60）, which was significantly higher than 76.7% （46/60） of the control group, and the difference was statistically significant（ χ^2 = 4. 016, P = O. 028）. Conclusion Thunderstorm moxibustion can effectively alleviate the upper limb spasm after stroke, and improve the Ashworth spasm scale spasticity score, clinical efficacy and patients＇ satisfaction.
