检验前过程对肝素结合蛋白、同型半胱氨酸及胃泌素释放肽前体检测结果的影响

摘　　要：目的比较不同样本类型、处理时间及保存条件的肝素结合蛋白(HBP)、同型半胱氨酸(Hcy)、胃泌素释放肽前体(Pro GRP)水平,探讨检验前过程对结果的影响。方法同源不同类型样本,经不同前处理后,采用酶联免疫吸附法检测HBP水平,双试剂循环酶法检测Hcy水平,Abbott全自动化学发光仪检测Pro GRP水平。结果与枸橼酸钠抗凝血浆HBP结果比较,EDTA-K2抗凝血浆结果显著降低(P<0.05),血清结果显著增高(P<0.05),且3种样本结果间无相关性。EDTA-K2抗凝血浆Hcy结果低于血清结果,上述差异均有统计学意义(P<0.05),随样本离心前放置时间的延长,血浆Hcy结果增高。血浆Pro GRP室温保存8 h,差异无统计学意义(P>0.05),2℃~8℃保存72 h结果略有下降,差异有统计学意义(P<0.05)。血清样本随放置时间的延长,结果降低,与血浆样本差异有统计学意义(P<0.05)。结论样本类型和前处理对HBP、Hcy和Pro GRP结果有影响,检验前工作的完善将进一步提高检验结果可靠性。Objective Through comparing the levels of heparin-binding protein（HBP）,homocysteine（Hcy） and pro-gastrin-releasing-peptide（Pro GRP） among different samples types,processing time and storage temperatures,to explore the influence of pre-examination processing on the accuracy of test results. Methods Different types of homologous samples underwent various pre-treatments,and the HBP level was detected by enzyme-linked immunosorbent assay（ELISA）,the Hcy level was detected by liquid stable 2-part enzymatic conversion method,and the Pro GRP level was detected using Abbott chemiluminescence reagent and automatic analyzer. Results Compared with HBP results of sodium citrate anticoagulant plasma,the results of EDTA-2 K anticoagulant plasma significant decreased（P〈0. 05）,and the serum results significant increased（P〈0. 05）,and there was no correlation in the three HBP samples. Hcy results of EDTA-2 K plasma were lower than the serum results,with the differences above statistically significant（P〈0. 05）. With the time extension before centrifugation,plasma Hcy significantly increased. Plasma Pro GRP was stored at room temperature for 8 hours,and the differences was not statistically significant（P〈0. 05）,and the results of plasma Pro GRP slightly decrease if stored at 2 ℃-8 ℃ for 72 hours,with the differences statistically significant（P〈0. 05）. With the time extension,the serum samples decreased continuously,and the difference to the plasma samples was statistically significant（P〈0. 05）. Conclusion The results of HBP,Hcy and Pro GRP are significantly influenced by the sample type and pre-treatment. The improvement of pre-examination processing will be an effective risk control to ensure the reliability of test results.
