摘 要:目的了解肿瘤患者下呼吸道感染病原菌分布及耐药性,为临床抗感染治疗提供科学依据。方法收集376份肿瘤患者下呼吸道感染的痰标本进行细菌培养及药敏鉴定。结果 376份痰标本,共分离出437株致病菌。革兰阴性菌为主要致病菌,其次为真菌、革兰阳性球菌。检出率占前5位的分别为白假丝酵母菌、肺炎克雷伯菌、大肠埃希菌、金黄色葡萄球、鲍曼不动杆菌。细菌药敏结果显示革兰阴性杆菌对亚胺培南及加酶抑制剂的抗菌药物较敏感,革兰阳性球菌对万古霉素及利奈唑烷高度敏感。结论肿瘤患者下呼吸道感染病原菌的耐药现象较严重,应加强细菌药敏监测以提高疗效和减少耐药株的产生。Objective To understand the distribution and drug resistance of pathogens causing lower respiratory tract infection in cancer patients,so as to provide scientific basis for clinical anti-infection treatment. Methods The sputum specimens from376 patients with lower respiratory tract infection were collected for bacterial culture and drug sensitivity test. Results 437 strains of pathogenic bacteria were isolated from 376 sputum specimens. Gram-negative bacteria was the main pathogen,followed by Fungi and Gram-positive cocci. The top five detection rates were of Candida albicans,Klebsiella pneumoniae,Escherichia coli,Staphylococcus aureus,Acinetobacter baumannii,and bacterial drug sensitive results showed that Gram-negative bacilli was sensitive to imipenem and enzyme inhibitors,and Gram-positive cocci was highly sensitive to vancomycin and linezolid. Conclusion The drug resistance of pathogens causing lower respiratory tract infection in cancer patients is severe,and the monitoring of bacterial drug sensitivity should be strengthened to improve the curative effect and reduce the production of drug resistant strains.
