探讨双胎妊娠并发妊娠期高血压疾病的高危因素

摘　　要：目的探讨双胎妊娠并发妊娠期高血压疾病的高危因素。方法选取2015年9月-2016年11月在我院常规产检并分娩的双胎妊娠并发妊娠期高血压疾病的孕妇38例作为观察组,另外选取同时期的双胎无妊娠期高血压的孕妇40例作为对照组。对母体特征、特性指标进行单因素和多因素非条件Logistic回归分析方法分析双胎妊娠并发妊娠期高血压的高危因素。结果多因素分析结果提示年龄≥35周岁(OR=5.025,95%CI:1.277-19.766)、初产(OR=17.0,95%CI:3.6-81.212)、人工辅助生育助孕(OR=4.68,95%CI:1.366-16.033)、孕前高BMI(OR=4,95%CI:1.468-10.898)、高血压疾病家族史(OR=17.0,95%CI:3.6-81.212)均为双胎妊娠并发妊娠期高血压疾病的高危因素,P<0.05,差异有统计学意义。结论高龄、初产、人工辅助生育、孕前高BMI、高血压家族史均为双胎并发妊娠期高血压疾病的高危因素。孕期应对具有高危因素的双胎孕妇加强健康教育和妊娠监督,改善母儿预后,减少严重不良妊娠结局的发生。Objective：To explore the high-risk factors of gemellary pregnancy combined with gestational hypertension. Methods：38 cases of pregnant women with gemellary pregnancy combined with gestational hypertension were selected from those who accepted regular production inspection and delivery in our hospital between September 2015 and November 2016 as the observation group,and 40 cases of peer pregnant women with gemellary pregnancy while without gestational hypertension were selected as the control group. Univariate analysis and multivariate analysis non-conditional Logistic regression was used to analyze the risk factors of gemellary pregnancy combined with gestational hypertension from maternal characteristics and characteristic indexes. Results：Multi-factor analysis results suggested that age ≥ 35 years（OR=5.025,95% CI：1.277-19.766）,primiparity（OR=17.0,95% CI：3.6-81.212）,artificial insemination（OR=4.68,95% CI：1.366-16.033）,the pregnancy high BMI（OR=4,95% CI：1.468-10.898）,family history of hypertension（OR=17.0,95% CI：3.6-81.212）were high-risk factors of gemellary pregnancy combined with gestational hypertension,with statistically significant differences（P〈0.05）. Conclusion：Advanced age,primiparity,artificial insemination,family history of hypertension are all high-risk factors of gemellary pregnancy combined with gestational hypertension. For gemellary pregnant women who have high risk factors,health education and pregnancy monitoring should be strengthened,and the prognosis of the mother and children should be improved to reduce the occurrence of severe adverse pregnancy outcomes.
