蠲痹历节清方治疗痛风性关节炎瘀热阻滞证31例临床观察

摘　　要：目的:观察蠲痹历节清方治疗急性痛风性关节炎瘀热阻滞证的临床疗效及其对关节滑液中MMP-9表达的影响。方法:将62例痛风性关节炎瘀热阻滞证患者随机分为2组各31例。治疗组采用蠲痹历节清方治疗,对照组采用依托考昔治疗,疗程均为15d。观察2组治疗前后临床症状及C反应蛋白(CRP)、红细胞沉降率(ESR)、血尿酸(UA)、血清白介素-8(IL-8)、MMP-9的变化,评定综合疗效。结果:总有效率治疗组为90.32%,对照组为83.87%,组间比较,差异有统计学意义(P<0.05);2组CRP、ESR、UA、IL-8、MMP-9治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05)。结论:蠲痹历节清方治疗急性痛风性关节炎瘀热阻滞证疗效显著。Objective： To investigate the clinical effect of Juanbi Lijieqing prescription in the treatment of acute gouty arthritis with heat stasis syndrome and its effect on the expression of matrix metallopeptidase-9（MMP-9） in synovial fluid. Methods： A total of 62 patients with gouty arthritis with heat stasis syndrome were randomly divided into treatment group and control group,with 31 patients in each group. The patients in the treatment group were given Juanbi Lijieqing prescription,and those in the control group were given etoricoxib. The course of treatment was 15 days for both groups. The two groups were compared in terms of the changes in clinical symptoms,C-reactive protein（CRP）,erythrocyte sedimentation rate（ESR）,serum uric acid（UA）,interleukin-8（IL-8）,and MMP-9 after treatment,and overall clinical outcome was evaluated. Results： There was a significant difference in overall response rate between the treatment group and the control group（90. 32% vs83. 87%,P〈0. 05）. Both groups had significant changes in CRP,ESR,UA,IL-8,and MMP-9 after treatment,and there were significant differences between the two groups（P〈0. 05）. Conclusion： Juanbi Lijieqing prescription has a good clinical effect in the treatment of acute gouty arthritis with heat stasis syndrome.
