摘 要:目的:运用气机理论选穴,研究针刺对糖尿病大鼠胰岛素受体结合率及葡萄糖转运蛋白4(GULT4)表达的影响。方法:雄性清洁级SD大鼠70只,随机抽取20只为正常组,其余单次腹腔注射链脲佐菌素(STZ,30mg/kg)复制糖尿病大鼠模型,造模成功共44只,随机分为两组:模型组与针刺组,各22只。针刺组针刺双侧天枢、双侧足三里、百会、双侧涌泉,15min/次,1次/d,治疗6d休息1d,疗程6周。治疗6周后,ELISA法检测大鼠血清胰岛素;Kilp法制备肝脏及腓肠肌细胞膜,Nishimura方法测定胰岛素结合率;RT-PCR检测大鼠骨骼肌组织GULT4基因表达。结果:与正常组比较,模型组FPG、2h PG、FINS、IRI均升高,胰岛素结合率、腓肠肌GLUT4蛋白及GLUT4 mRNA的表达水平明显降低(P<0.05);治疗后,针刺组与模型组比较,FPG、2h PG、FINS、IRI均明显降低,胰岛素结合率、GLUT4蛋白及GLUT4 mRNA的表达水平显著升高(P<0.05)。结论:针刺天枢、足三里、百会、涌泉,可以降低糖尿病大鼠的血糖及胰岛素水平,提高胰岛素受体结合率,在分子水平上影响GLUT4的表达。也为进一步研究"升阳(气)抑阳(热)"治法,为开拓气机理论在糖尿病的防治应用提供实验数据支持。Objective: To investigate the effects of acupuncture on insulin receptor binding rate and GULT4 expression in diabetic rats based on theory of qi circulation. Methods: Seventy male clean SD rats were randomly selected for trial and 20 of which were randomly selected for normal group, the others were singly intraperitoneal injected with STZ(30 mg/kg) to replicate the diabetic rat model. The models were randomly divided into two groups: the model group(n=22) and the acupuncture group(n=22). The acupuncture group was treated with needling at Tianshu(ST25), Zusanli(ST36), Baihu(DU20), Yongquan(KI1), for 15 min once a day, six times a week, the treatment lasted for six weeks. ELISA was used to detect serum insulin in rats. The membrane of liver and gastrocnemius muscle was prepared by Kilp, and Nishimura method was used to determine the insulin binding rate. RT-PCR was used to detect the expression of glucose transporter 4 gene in skeletal muscle tissue of rats. Results: Compared with the normal group, FPG, 2 h PG, FINS, IRI, insulin binding rate and the expression of GLUT4 mRNA were significantly decreased in the model group(P〈0.05). Compared with the the model group after the treatment, FPG, 2 h PG, FINS, and IRI in acupuncture group were significantly reduced, the insulin binding rate and expression of GLUT4 mRNA were significantly increased(P〈0.05). Conclusion: It can reduce the blood glucose and insulin level of diabetic rats, and improve the binding rate of insulin receptor affect the expression of GLUT4 at the molecular level by acupuncturing Tianshu(ST25), Zusanli(ST36), Baihui(DU20), Yongquan(KI1). It also can provide experimental data support for further study on ascending qi and inhibiting heat in treatment of DM.
