摘 要:目的探讨在聚甲基丙烯酸甲酯骨水泥(PNMA)中加入不同剂量的生理盐水,对椎体成形术后骨折椎体生物力学的影响。方法选取防腐尸体标本6具,年龄为(65±2)岁,取T11~L4椎体,选取骨密度相近的32个椎体,均分为A、B、C、D4组,将每组椎体制成压缩骨折模型,同时记录最大垂直载荷及刚度,将混有0、0.5、1.0、1.5ml的生理盐水注入5ml PMMA骨水泥中,混合均匀后,从中抽取3m1分别注入A、B、C、D4组椎体中,4组椎体行生物力学实验,记录最大垂直载荷及刚度,应用SPSS21.0统计软件分析。结果对4组椎体术前、术后最大垂直载荷进行统计学分析,A组:P=0.000,B组:P=0.000,C组:P=0.000,D组:P=0.255;对4组椎体术前、术后的刚度进行统计学分析,A组:P=0.000,B组:P=0.008,C组:P=0.028,D组:P=0.375,A、B、C3组与术前比较差异有统计学意义,D组差异无统计学意义。结论术后各组椎体的最大垂直载荷及刚度均有所增加,D组与术前接近。Objective To investigate the effect of adding different doses of normal saline in polymethylmethacrylate (PMMA) bone cement on the biomechanics of fractured vertebral body after vertebroplasty. Methods Thirty - two vertebral bodies were harverted from 6 embalmed cadavers, and vertebral bodies were divided into groups A, B, C and D. All specimens were made into compression fracture model by the biomechanical machine. The maximum vertical load and stiffness were recorded simultaneously. The normal saline (0, 0. 5, 1.0, and 1.5 ml) was injected into 5 ml of PMMA bone cement. After mixing, 3 ml of bone cement was injected into the vertebrae of groups A, B, C and D, respectively, and the vertebrae of four groups were subjected to biomechanical experiment. The maximum vertical load and stiffness were recorded, and statistical analysis was carried out by SPSS 21.0 software. Results The maximum vertical load in the four groups before and after operation was analyzed statistically ( A : P = 0. 000 ; B : P = 0. 000; C:P =0. 000; D:P =0. 255). The stiffness in the four groups before and after operation was analyzed statistically ( A : P = 0. 000 ; B : P = 0. 008 ; C : P = 0. 028 ; D : P = 0. 375 ). After operation, the vertebral bodies in groups A, B and C were statistically significantly different from those before operation, and there was no statistically significant difference in group D before and after operation. Conclusion The maximum vertical load and stiffness of all vertebral bodies increased after surgery. There was no significant difference in group D before and after vertebral surgery.
