摘　　要：目的:探讨临床药师参与临床制订用药方案的药学实践方法及其体会。方法:临床药师监护患者用药全过程,针对患者病情变化给出用药合理化建议。结果与结论:临床药师通过参与临床治疗团队,优化了给药方案以及全过程的药学监护,避免了患者药物不良反应的发生。Objective： To explore the pharmaceutical practice of clinical pharmacists participating in the formulation of clinical medication plans and their experience.Methods： Clinical pharmacists monitored the whole process of patients＇ medication.The reasonable advices on medication for the change of the patient＇s condition were given.Results and conclusion： The drug delivery program was optimized,and in the whole process of pharmaceutical monitoring the occurrence of adverse drug reactions was avoided.
