参芪化浊饮对脾肾气虚湿浊型慢性肾衰竭患者中医证候及肾功能指标的影响

摘　　要：目的：探讨参芪化浊饮对脾肾气虚湿浊型慢性肾功能衰竭患者中医证候及肾功能指标的影响。方法：选取自2015年1月—2016年12月我院住院治疗的慢性肾功能衰竭患者78例,参照病例对照原则,随机分为对照组39例和实验组39例,在常规治疗基础上,对照组采用金水宝胶囊治疗;实验组在对照组的基础上采用参芪化浊饮加减治疗,两组共持续4周为1个疗程。治疗前后采肘静脉血,评定两组患者中医症状积分,检测血肌酐、血尿素氮等肾功能指标,同时对比两组间临床疗效,记录两组间不良反应事件。结果：两组患者治疗后常见中医症状如倦怠乏力、腰膝酸软、恶心呕吐、水肿等明显减轻或消失,治疗组明显优于对照组（P〈0.05）;两组患者治疗后Scr、BUN较治疗前明显降低,且治疗组降低更为明显（P〈0.05）;对照组总有效率74.36%低于实验组总有效率92.31%,有差异（P〈0.05）。结论：参芪化浊饮对脾肾气虚湿浊型慢性肾功能衰竭患者的疗效确切,有助于提高肾功能,改善临床症状,延缓肾衰竭的进程,且不增加药物的不良反应,是一种有效的治疗方案。Objective：To discuss the effect of Shenqi Huazhuo Yin on the syndrome of traditional Chinese medicine and renal function target of the patients with chronic renal failure of spleen,kidney Qi deficiency and humid turbid type. Methods：78 patients with chronic renal failure in our hospital from January 2015 to December 2016 were selected. According to the principle of case-control,they were randomly divided into 39-case in control group and 39-case in experimental group. Based on routine treatment,the control group was treated with Jinshuibao Capsule and the experimental group was treated with Shenqi Huazhuo Yin addition and subtraction on the basis of the control group,4 weeks as one course of the treatment. The venous blood was collected,before and after treatment. TCM symptom scores of the two groups were evaluated,and renal function indicators such as serum creatinine and blood urea nitrogen were measured. Clinical efficacy was also compared between the two groups and adverse reaction events between the two groups were recorded. Results：TCM symptoms such as fatigue and lack of strength,soreness and weakness of waist and knees;vomiting,edema and others,were significantly reduced or disappeared after treatment in both groups. The treatment group was significantly better than the control group（P〈0.05）;Scr,BUN were significantly lower than those before treatment,and the treatment group decreased more significantly（P〈0.05）;The total effective rate of the control group was 74.36%,which was lower than the experimental group,the total efficiency of which was 92.31%. There were differences between two groups（P〈0.05）. Conclusion：Shenqi Huazhuo Yin has a definite curative effect on the patients with chronic renal failure of spleen,kidney Qi deficiency and humid turbid type. It is helpful to improve renal function,clinical symptoms,and delay the progression of renal failure,which does not increase adverse drug reactions as well. It＇s an effective treatment plan.
