幽门螺旋杆菌相关性胃炎中医体质分布规律研究

摘　　要：目的探讨幽门螺旋杆菌相关性胃炎的中医体质分布规律,为中医辨治幽门螺旋杆菌相关性胃炎提供参考。方法选择197例经胃镜诊断幽门螺旋杆菌相关性胃炎患者,根据"中医体质分类与判定表"判定其中医体质类型,并观察不同中医体质分布情况,并比较不同体质患者的胃镜及组织病理学表现。结果 197例患者中,平和质22例(11.17%)、气虚质17例(8.63%)、阳虚质32例(16.24%)、阴虚质11例(5.58%)、痰湿质23例(11.68%)、湿热质46例(23.35%)、血瘀质8例(4.06%)、气郁质38例(19.29%)、特禀质0例(0%),其中,湿热质患者最为多见。不同中医体质胃镜征象及组织病理学表现存在差异,其中,湿热质伴糜烂最多见,阳虚质、气虚质的萎缩程度较重,气虚质、湿热质的肠化程度较重。结论幽门螺旋杆菌相关性胃炎中医体质类型主要为湿热质、气郁质、阳虚质、痰湿质。临床根据幽门螺旋杆菌相关性胃炎的中医体质分布特点,"辨体-辨病-辨证"三辨施治幽门螺旋杆菌相关性胃炎,可提高幽门螺旋杆菌相关性胃炎的治愈率并降低复发率,对预防幽门螺旋杆菌感染起到积极作用。Objective To investigate the distribution of traditional Chinese medicine constitution in Helicobacter pylori-related gastritis.Method 197 patients with the Helicobacter pylori-related gastritis were selected and assessed by the constitutional type and the distribution of 197 the Helicobacter pylori-related gastritis patients to the table of TCM constitution classification and decision. By observing the gastroscopoic and pathological performance,the correlation between the body constitution of TCM and the gastroscope and pathological performance was compared. Results Among these patients,the patients with constitution of gentler,qi-deficiency,yang-deficiency,yindeficiency,blood stasis,phlegm dampness,dampness heat,depressed qi and specific endowment accounted for 11. 17%,8. 63%,16. 24%,5. 58%,11. 68%,23. 35%,4. 06%,19. 29% and 0%,respectively. Comparing with the other constitution,the patients with constitution of dampness heat are most. Gastroscopoic and pathological manifestations have a bearing on different body constitution of TCM types. Conclusion Dampness heat,depressed qi,yang-deficiency,dampness are the major types of the body constitution of TCM of the Helicobacter pylori-related gastritis. It is indicate that treatment of the Helicobacter pylori-related gastritis should combine the body constitution of TCM,base on adjusting the patients body constitution,and we can improve the curative effect of the TCM,reduce the recurrence,and prevent the disease.
