医养结合居家医护服务体系的构建与管理

摘　　要：目的建立居家医护服务体系,实现居家健康养老。方法建立居家医护服务培训中心,对杭州市江干区的医务人员进行居家医疗、护理知识及技能的培训;建立居家医护服务管理中心,制订相关制度、职责、作业流程、服务项目、考核评价标准等,派遣社区医务人员为老年人提供居家护理、居家康复、用药咨询、居家营养服务;与原有智慧养老服务平台对接,互为补充,逐步完善医养结合服务体系。结果 2015年1月—2017年8月,管理中心共接收服务申请1 433例,建档1 344例,提供各类型居家医护服务10 408例次,其中居家护理1 059例次,上门检验189例次,居家康复198例次,居家营养和居家用药咨询2例次,患者总满意度达90%以上。结论居家医护服务体系的建立,一定程度上减轻了居民多次往返医院进行延续护理、康复锻炼的负担,使老年人在家便可获得优质的医疗、护理和养老服务,服务满意度较高。Objective To construct home care service system and to realize elderly care at home. Methods We established a training center for home care service,providing training focusing on medical treatment at home,nursing knowledge and skills among health care providers from one district in Hangzhou. We established a management center for home care service,and formulated relevant regulations,responsibilities,operation procedure,service items and evaluation standards. Community health care providers were sent to provide home care service for the elderly, including rehabilitation,medication consultation,and nutrition service. We also cooperated with the existing aged service platform in the city to get complementary and comprehensive development. Results Since January 2015,the management center received 1 433 application cases and implemented 1 344 cases,provided services to 10408 cases, including home care 1 059 cases,door-to-door test 189 cases,home rehabilitation 198 cases,home nutrition and pharmacy 2 cases. Patient satisfaction was higher than 90%. Conclusion The home care service system can reduce the burden of medical care for the elderly and provide high quality medical service to the elderly at home with high satisfaction from the elderly.
