皮-罗序列征患儿胃管留置方法的改良

摘　　要：目的改良皮-罗序列征气道狭窄患儿留置胃管的方法 ,并评价效果。方法选取皮-罗序列征患儿81例,观察组52例采用改良后的胃管留置方法,置管过程中将患儿头向后仰呈轻度仰伸位并偏向一侧,向前抬起下颌,解除了舌后坠及气道堵塞,进行置管;对照组29例采用传统的胃管留置方法。比较两组的一次性置管成功率、不良反应发生率及平均置管时间。结果观察组与对照组的一次性置管成功率分别为80.7%与27.5%,胃管盘曲发生率分别为3.8%与44.8%;观察组不良反应发生率为窒息1.9%、恶心/呕吐7.6%、无黏膜出血发生,对照组为窒息24.1%、恶心/呕吐51.7%、黏膜出血10.3%;置管时间观察组为3(2,4.75)min,对照组为10(7.5,16)min,差异均具有统计学意义(均P<0.05)。结论改良留置胃管法可以提高皮-罗序列征患儿留置胃管的一次性成功率,缩短置管时间,减少患儿在置管过程中不良反应的发生。Objective To explore the effects of modified indwelling gastric tube method in infants with Pierre Robin sequence. Methods A total of 81 infants with Pierre Robin sequence were included. The observation group （52 cases） was provided with modified gastric tube retention method：tilting the head back to a slightly elevated position,slanting the head to one side and lifting the jaw forward to relieve glossoptosis and upper airway obstruction. The control group （29 cases） was provided with traditional gastric tube retention method. Data of one-time success rate,incidence of adverse events and intubation time were gathered. Results The one-time success rate for the observation group and the control group was 80.7% and 27.5%,and the incidence of tube bending was 3.8% and 44.8%. The adverse events in the observation group were apnea（1.9%）,nausea/vomiting（7.6%）,and no mucosal bleeding occurred. The adverse events in the control group were apnea （24.1%）,nausea/vomiting （51.7%）,and bleeding（10.3%）; mean intubation time was 3（2,4.75） min in the observation and 10（7.5,16） min in the control group,and the difference was statistically significant （P〈0.05）. Conclusion The modified gastric tube retention method can improve one-time success rate,shorten intubation time,and reduce the occurrence of adverse events in infants with Pierre Robin sequence.
