半剂量瑞替普酶联合瑞舒伐他汀对急性ST段抬高型心肌梗死患者凝血状态的影响

摘　　要：【目的】探讨半剂量瑞替普酶联合瑞舒伐他汀对急性ST段抬高型心肌梗死（STEMI）患者凝血状态的影响。【方法】选取2016年10月至2017年10月我院收治的急性STEMI患者86例作为研究对象，随机分为对照组（n=43，半量瑞替普酶治疗）与观察组（n=43，半量瑞替普酶联合瑞舒伐他汀治疗），观察两组心功能、凝血因子及血清因子等指标。【结果】治疗后，观察组左心室舒张末期内径（LVEDD）、左心室收缩末期内径（LVESD）明显低于对照组（P〈0．05），心搏出量（SV）和左心室射血分数（LVEF）明显高于对照组（P〈0．05）。治疗后，观察组超敏C反应蛋白（hs-CRP）、同型半胱氨酸（Hcy）及补体3（C3）水平明显低于对照组（P〈0．05）。治疗后，观察组活化部分凝血活酶时间（APTT）、凝血酶原时间（PT）、凝血酶时间（TT）明显长于对照组（P〈0．05），纤维蛋白原（FIB）水平明显低于对照组（P〈0．05）。【结论】半剂量瑞替普酶联合瑞舒伐他汀治疗急性STEMI能够有效改善患者的心功能及凝血功能，同时降低血清因子hs-CRP、Hcy、C3水平。[ObjectivelTo investigate the effect of half-dose reteprase combined with resuvastatin on coagula- tion status in patients with acute ST-segment elevation myocardial infarction （STEMI）.[MethodslA total of 86 pa- tients with acute STEMI admitted in our hospital from October 2016 to October 2017 were randomly divided into two groups： control group （nong43, half dose of reteprase） and observation group （nyang43, half-dose of rete- prase combined with resuvastatin）, and the cardiac function of the two groups was observed. Coagulation factor and serum factor.[ResultslAfter treatment, left ventricular end-diastolic diameter （LVEDD） and left ventricular end-systolic diameter （LVESD） in the observation group were significantly lower than those in the control group （ P〈0.05）, and the cardiac output （SV） and left ventricular ejection fraction （LVEF） were significantly higher in the observation group than in the control group （ P 〈0.05）. After treatment, the levels of hs-CRP, homocysteine （Hcy） and complement 3 （C3） in the observation group were significantly lower than those in the control group （ P〈0.05）. After treatment, the activated partial thromboplastin time （APTT）, prothrombin time （PT） and thrombin time （TT） in the observation group were significantly longer than those in the control group （ P〈0. 05）, and the level of fibrinogen （FIB） was significantly lower than that in the control group （ P〈0.05）.[Conclu- sionlHalf-dose reteprase combined with resuvastatin in the treatment of acute STEMI can effectively improve the cardiac function and coagulation function of the patients, and reduce the level of serum hs-CRP, Hey and C3in pa- tients with acute STEMI.
