当归芍药散联合异甘草酸镁治疗原发性胆汁性肝硬化的疗效观察及对血清MMP2、MMP9、VEGF水平的影响

摘　　要：[目的]探讨当归芍药散联合异甘草酸镁治疗原发性胆汁性肝硬化的疗效观察及对血清基质金属蛋白酶2(MMP2)、基质金属蛋白酶9(MMP9)、血管内皮生长因子(VEGF)水平的影响。[方法]选取符合研究条件的110例患者随机分为观察组和对照组,每组55例。2组患者均接受常规治疗,对照组加用异甘草酸镁,观察组在对照组的基础上内服当归芍药散,比较2组治疗后的临床症状、肝功能指标、免疫球蛋白及血清MMP2、MMP9、VEGF水平的变化。[结果]治疗后,2组患者口干、皮肤瘙痒、黄疸及乏力的发生率均明显低于治疗前,观察组发生率显著低于对照组,差异均有统计学意义(均P<0.05);观察组的总有效率(87.27%)明显高于对照组(67.27%)(P<0.05);2组治疗后的天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、碱性磷酸酶(ALP)、血清总胆红素(TBiL)及γ-谷氨酰胺转肽酶(γ-GGT)水平显著低于治疗前,且观察组显著低于对照组,差异均有统计学意义(均P<0.05);治疗后,2组患者的免疫球蛋白A(IgA)、免疫球蛋白G(IgG)及免疫球蛋白M(IgM)、MMP2、MMP9和VEGF水平均显著低于治疗前,且观察组显著低于对照组,差异均有统计学意义(均P<0.05);2组不良反应的发生率无明显差异(P>0.05)。[结论]当归芍药散联合异甘草酸镁治疗原发性胆汁性肝硬化能较好地改善临床症状与肝功能,提高免疫力,改善肝纤维化,消减肝腹水,具有较好的临床疗效。[Objective]To investigate the curative effect of Dangguishaoyao powder combined with Magnesium Isoglycyrrhizinate in the treatment of primary biliary cirrhosis and the influence on serum MMP2,MMP9 and VEGF levels.[Methods]One hundred and ten eligible participants were randomly divided into the observation group and the control group,fifty-five cases in each group.Two groups were treated with conventional treatment,on this basis,the control group added Magnesium Isoglycyrrhizinate,the observation group taken the Dangguishaoyao powder for oral treatment on the basis of control group.The clinical symptoms,liver function index,immunoglobulin and serum matrix metalloproteinase 2（MMP2）,matrix metalloproteinase 9（MMP9）and vascular endothelial growth factor（VEGF）levels were compared between the two groups after treatment.[Results]After treatment,the incidences of dry mouth,pruritus,jaundice and fatigue in the two groups was significantly lower than before treatment（P〈0.05）,and the incidence rate in the observation group was significantly lower than that in the control group（P〈0.05）;the total effective rate of the observation group was 87.27%,which was significantly higher than that of the controlgroup 67.27%（P〈0.05）;the levels of AST,ALT,ALP,TBiL andγ-GGT in the observation group and the control group were significantly lower than those before treatment（P〈0.05）;and the observation group was significantly lower than the control group（P〈0.05）;after treatment,the levels of IgA,IgG and IgM in the two groups were significantly lower than those before treatment（P〈0.05）,the observation group was significantly lower than the control group（P〈0.05）;after treatment,serum MMP2,MMP9 and VEGF levels in the two groups were significantly lower than those before treatment（P〈0.05）,while serum MMP2,MMP9 and VEGF levels in the observation group were significantly lower than those in the control group（P〈0.05）;there was no significant difference in the incidence of adverse rea
