摘  要:目的检测新诊断多囊卵巢综合征(PCOS)患者血清正五聚蛋白3(PTX3)和内脏脂肪特异性丝氨酸蛋白酶抑制剂(vaspin)水平,探讨其与PCOS不同表型的相关性。方法入选遵义医学院附属医院内分泌科、生殖中心新诊断PCOS患者75例,另外选择健康对照者70名。根据体重指数分为非肥胖PCOS组(体重指数〈23kg/m2,n=33)、超重/肥胖PCOS组(体重指数≥23kg/m2,n=42)、非肥胖对照组(体重指数〈23kg/m2,n=34)、超重/肥胖对照组(体重指数≥23kg/m2,n=36)。检测空腹血糖、甘油三酯、总胆固醇、高密度脂蛋白-胆固醇(HDL-C)、低密度脂蛋白-胆固醇(LDL-C)、空腹胰岛素(FINS)、雌二醇、硫酸脱氢表雄酮(DHEAS)、卯泡刺激素(FSH)、黄体生成素(LH)、孕酮、催乳素(PRL)、总睾酮等指标。ELISA测定血清PTX3、vaspin水平。采用稳态模型评估-胰岛素抵抗指数(HOMA-IR)评价胰岛素抵抗程度,并与PTX3、vaspin进行相关分析。结果与非肥胖对照组相比,非肥胖PCOS组血清PTX3水平下降,vaspin水平升高(F=3.446、6.653,P均〈0.05);超重/月巴胖PCOS组血清vaspin水平升高(F=4.125,P〈0.05)。与非肥胖PCOS组相比,超重/肥胖对照组血清vaspin水平升高(F=6.238,P〈0.05)。与超重/肥胖对照组相比,超重/肥胖PCOS组血清PTX3水平下降(F=5.273,P〈0.01),vaspin水平升高(F=7.124,P〈0.05)。血清PTX3与空腹血糖、甘油三酯、FINS、HOMA-IR、DHEAS、总睾酮呈负相关(r=-0.172、-0.190、-0.294、-0.274、-0.236、-0.243,P均〈0.05),与年龄呈正相关(r=0.240,P〈0.05)。Vaspin与体重指数、HOMA—IR、总胆固醇呈正相关(r=0.331、0.418、0.172,P均〈0.05)。结论PCOS患者血清F1X3水平降低、vaspin水平升高,其水平变化可能与PCOS患者体内高雄激素血症及胰岛素抵抗Objective To determine the levels of serum pentraxin 3 ( PTX3 ) and visceral adipose tissue-derived serine protease inhibitor(vaspin) in newly-diagnosed polyeystie ovary syndrome (PCOS) pa- tients, and investigate the relationship between serum level of PTX3, vaspin and different phenotypes of PCOS. Methods Seventy five patients with newly-diagnosed PCOS and 70 age and body mass index (BMI)-matched healthy controls who visited the Department of Endocrinology and Reproduction Centre of the Affiliated Hospital of Zunyi Medical University were enrolled in the study. According to their BMI, the subjects were divided into non-obese PCOS group ( BMI 〈 23 kg/m2, n = 33 ) , overweight or obese PCOS group ( BMI≥ 23 kg/m2, n = 42 ), non-obese control group ( BMI 〈 23 kg/m2, n = 34 ) and overweight or obese control group ( BMI ≥ 23 kg/m2, n = 36 ). Fasting plasma glucose ( FPG), triglyeeride ( TG), total cholesterol( TC), high density lipoprotein-cholesterol ( HDL-C ), low density lipoprotein-cholesterol ( LDL-C), fasting plasma insulin ( FINS ), etradiol ( E2 ), dehydroepiandrosterone ( DHEAS ), follicle stimulating hormone ( FSH), luteinizing hormone ( LH), progesterone, prolactin (PRL) and total testosterone were measured. Serum PTX3 and vaspin levels were detected with ELISA. The homeostasis model assessment of insulin re- sistance(HOMA-IR) was used to estimate insulin resistance and its correlations with PTX3 and vaspin level were analyzed. Results Compared with non-obese control group, the level of PTX3 was reduced,while the level of vaspin was elevated in non-obese PCOS group ( F = 3. 446,6. 653, all P 〈 0.05 ). The level of vaspin was elevated in overweight or obese PCOS group (F = 4. 125, P 〈 0.05 ). Compared with non-obese PCOS group, the level of vaspin was elevated in overweight or obese PCOS group ( F = 6. 238, P 〈 0.05 ).
