摘 要:目的探讨CT在先天性心血管疾病(CCD)诊断中的应用价值。方法对75例CCD患者的CT与超声心动图影像学资料进行回顾性分析,将其诊断结果与病理诊断结果对照,比较两组诊断方法的准确率与畸形检出率。结果 75例CCD患者术后病理诊断结果为:心脏畸形63处,大血管畸形以及心脏和血管连接部位畸形68处,总共131处。CT检出复杂、复合畸形57例,单发畸形17例,诊断准确率为98.67%(74/75),心脏畸形检出60处,大血管畸形以及心脏和血管连接部位畸形67处,畸形检出率分别为95.24%(60/63)、98.53%(67/68)。超声心动图检出心血管内畸形72例,诊断准确率为96.00%(72/75),心脏畸形检出62处,大血管畸形以及心脏和血管连接部位畸形65处,畸形检出率分别为98.41%(62/63)、95.59%(65/68)。两种检查方式诊断准确率、畸形检出率比较,差异无统计学意义(P>0.05)。结论采用多层螺旋CT诊断CCD具有准确率、畸形检出率高等优点,值得临床进一步推广应用。Objective To investigate the application value of CT in the diagnosis of congenital cardiovascular disease(CCD). Methods The data of CT and echocardiographic of 75 patients with CCD who were retrospectively analyzed. The diagnostic results were compared with those of pathological diagnosis. The accuracy and deformity detection rate of two diagnostic methods were compared. Results The pathological diagnosis results of 75 CCD patients were as follows: 63 parts of cardiac malformations, 68 parts of macrovascular malformations and malformations of cardiac and vascular connections, a total of 131 parts. Among them, 57 cases of complicated and complex deformities were detected by multi-slice spiral CT, 17 cases were diagnosed as single deformity, the diagnostic accuracy rate was 98.67%(74/75), 60 parts of cardiac malformations, 67 parts of macrovascular malformations and deformities of heart and vascular connections. The detection rates of deformities were 95.24%(60/63), 98.53%(67/68) respectively.72 cases of cardiovascular malformations examined by echocardiography, the diagnostic accuracy rate was 96.00%(72/75), 62 parts of cardiac malformations, 65 parts of macrovascular malformations and deformities of heart and vascular connections, the detection rates of deformities were 98.41%(62/63), 95.59%(65/68) respectively.There was no significant difference between two test methods in the accuracy and deformity detection rate(P〉0.05). Conclusion The multi-slice spiral CT has the advantages of high accuracy and detection rate of deformity, it is worthy of further clinical popularization and application.
