肉芽肿性乳腺炎手术时机的选择及其对临床治疗效果的影响

摘　　要：目的探讨肉芽肿性乳腺炎手术时机的选择以及对临床疗效的影响。方法 45例肉芽肿性乳腺炎患者作为研究对象,患者均采用中药内服外敷治疗,选择合适的时间进行乳腺坏死病灶清除术进行治疗,观察其效果。结果急性肿块期患者治疗总有效率为100.00%(25/25),复发率为0(0/25);慢性肿块期患者治疗总有效率为81.82%(9/11),复发率为18.18%(2/11);脓肿期患者治疗总有效率为77.78%(7/9),复发率为22.22%(2/9);急性肿块期患者总有效率明显高于慢性肿块期患者、脓肿期患者,差异具有统计学意义(P<0.05)。急性肿块期患者术后复发率明显低于慢性肿块期患者、脓肿期患者,差异具有统计学意义(P<0.05)。结论对肉芽肿性乳腺炎患者选择正确的手术时机具有很大必要性,可有效降低肉芽肿性乳腺炎术后复发率,同时采取中西医结合的治疗方式可大大提升治疗效果。Objective To explore the choice of timing of surgery for granulomatous mastitis and its effect on the clinical treatment. Methods 45 patients with granulomatous mastitis were selected selected as the research objects. All patients were treated with internal and external application of traditional Chinese medicine, and the appropriate time was selected for the treatment of breast necrosis. The treatment effect was observed. Results The total effective rate of patients with acute mass was 100.00%（25/25）, the recurrence rate was 0（0/25）. The total effective rate of patients with chronic lump was 81.82%（9/11）, the recurrence rate was 18.18%（2/11）. The total effective rate of patients with abscess was 77.78%（7/9）, the recurrence rate was 22.22%（2/9）. The total effective rate of patients with acute mass was significantly higher than chronic lump patients and abscess patients, and the recurrence rate was significantly lower than chronic lump patients and abscess patients, the differences were statistically significant（P〈0.05）. Conclusion For patients with granulomatous mastitis, it is very necessary to choose the right surgery time. It can effectively reduce the recurrence rate of granulomatous mastitis. Meanwhile, the combination of traditional Chinese and Western medicine can greatly improve the effective rate of treatment.
