分娩球配合自由体位及间苯三酚对初产妇第一产程和分娩结局的影响

摘　　要：目的:探讨分娩球配合自由体位及间苯三酚对初产妇第一产程和分娩结局的影响。方法:选取2017年1-6月本院收治的初产妇200例。根据产妇意愿将其分为常规组(n=118)与研究组(n=82)。常规组采用传统卧位或半卧位待产分娩,研究组采用分娩球配合自由体位及间苯三酚待产分娩。比较两组第一、二、三产程所需时间,记录宫口开全平均时间,比较两组阴道分娩、宫颈水肿、宫颈裂伤等发生率,比较两组新生儿Apgar评分、窒息发生率。结果:研究组第一产程、第二产程、第三产程时间分别为(484.7±89.7)、(34.9±7.2)、(5.3±1.2)min,均显著低于常规组的(576.4±98.2)、(46.8±9.3)、(6.4±1.2)min,比较差异均有统计学意义(P<0.05);研究组宫口全开时间、阴道分娩率及子宫颈水肿发生率均显著少于常规组,比较差异均有统计学意义(P<0.05)。结论:分娩球配合自由体位及间苯三酚可有效缩短初产妇第一产程,提高其阴道分娩率,改善妊娠结局。Objective：To explore the effects of delivery ball combined with free position and Phloroglucinol on the first stage of labor and delivery outcomes of primiparas.Method：A total of 200 primiparas were treated in our hospital from January to June 2017 were selected.According to the maternal intention,they were divided into routine group（n=118） and study group（n=82）.The routine group was given traditional supine or semi supine for childbirth,and study was given the delivery ball combined with free position and Phloroglucinol for childbirth.The time of first stage of labor,second stage of labor and third stage of labor between two groups were compared,the average time of cervix full open was recorded,the incidence rates of vaginal delivery,cervical edema and cervical laceration between two groups were compared,the neonatal Apgar score and the incidence rate of asphyxia between two groups were compared.Result：The time of first stage of labor,second stage of labor and third stage of laborin study group were respectively（484.7±89.7）,（34.9±7.2）,（5.3±1.2） min,which were significantly lower than（576.4±98.2）,（46.8±9.3）,（6.4±1.2） min in routine group,the differences were statistically significant（P〈0.05）.The average time of cervix full open,the rate of vaginal delivery and the incidence of edema of the cervix in study groupwere significantly less than those of routine group,the differences were statistically significant（P〈0.05）.Conclusion：Delivery ball combined with free position and Phloroglucinol can effectively shorten the first stage of labor of primiparas,and improve the vaginal delivery rate and the pregnancy outcomes.
