封闭式负压吸引联合中药和碱性成纤维因子灌洗提高Ⅲ、Ⅳ期压疮疗效的研究

摘　　要：目的:研究封闭式负压吸引(VSD)联合中药和碱性成纤维因子(b-FGF)灌洗对Ⅲ、Ⅳ期压疮治疗效果。方法:选取本院住院患者中带入或新发的Ⅲ、Ⅳ期压疮患者57例(创面共69个)。按照不同治疗方法将创面其分为对照组25个、VSD组17个、联合组27个。对照组采用常规创面护理,VSD组采用VSD,联合组采用VSD联合中药、b-FGF灌洗。比较各组治疗前和治疗后第8天的创面面积及肉芽组织微血管计数(MVD)。结果:治疗后第8天,联合组的压疮面积缩小差值、MVD增加差值均优于对照组和VSD组,比较差异均有统计学意义(P<0.001)。结论:VSD联合中药、b-FGF灌洗能增加压疮肉芽组织中毛细血管密度,促进肉芽组织快速生长,创面愈合加快。Objective：To investigate the effect of vacuum-sealing drainage（VSD） combined with traditional Chinese medicine and basic fibroblast growth factor（b-FGF） irrigation on stage Ⅲ and Ⅳ pressure ulcer（PU）.Method：A total of 57 cases（69 wounds） of hospitalized patients with or new PU patients with stage Ⅲ and Ⅳ in our hospital were selected.According to different treatment methods,the wounds were divided into 25 wounds of control group,17 wounds in VSD group and 27 wounds in combined group.The control group was treated with conventional wound care,VSD group was treated with VSD,and combined group was treated with VSD combined with traditional Chinese medicine and b-FGF.The area of wound surface and microvascular count（MVD） of granulation tissue before and after treatment 8 days were compared.Result：After treatment 8 days,thedifference of wound surface area reduction and MVD increase in combined group were better than those of control group and VSD group,the differences were statistically significant（P〈0.001）.Conclusion：VSD combined with traditional Chinese medicine and b-FGF irrigation can increase the capillary density in granulation tissue of pressure sore,promote the rapid growth of granulation tissue and accelerate the healing of the wound.
