摘 要:目的:探讨磁共振(magnetic resonance imaging,MRI)在应力性骨折中的临床诊断价值。方法:分析我部野战医疗所2015~2016年收治的12例下肢应力性损伤患者的影像学诊断资料,所有患者均行X线片及MRI检查,分析诊断结果,选择下肢应力性骨折的最佳影像学诊断方法。结果:12例患者伤后2~3周X线片均未见明显异常;同期行MRI检查可见明显骨髓水肿,T1、T2加权像均可见线状低信号影。伤后4~6周X线片可见明显骨痂形成,证实骨折。结论:下肢应力性骨折主要发生在股骨髁、胫骨上端、胫骨远端、距骨、跟骨、跖骨等部位,伤后X线片难以诊断,但MRI显示明显骨折影像特征,能有效提高诊断准确率,避免误诊。Objective:To evaluate the clinical value of magnetic resonance imaging(MRI)in the diagnosis of stress fracture.Methods:The imaging diagnostic data of twelve cases with stress injury in the field medical center from 2015 to 2016 were analyzed.All patients were examined by X-ray and MRI,and the diagnostic effect were analyzed.The best imaging diagnosis method of stress fracture was choosen.Results:All the patients showed no abnormal X-ray within 2 to3 weeks after injury.MRI examination showed obvious linear low signal of bone marrow edema in both T1-weighted and T2-weighted image at the same period.After 4 to 6 weeks of injury,X-ray images were implemented and showed visible callus formation,confirming the existence of previous fracture.Conclusion:Lower extremity stress fractures occur mainly in the femoral condyle,upper part of tibia,distal part of tibia,talus,calcaneus and metatarsal.X-ray image is less sensitive to diagnose it.MRI can show typical fracture features and can significantly improve the diagnostic accuracy of stress fracture.
