摘　　要：目的了解甘肃省兰州市城关区门诊病人疾病谱的变化特点,为当地医疗卫生资源的合理配置提供参考依据。方法按照ICD-10对疾病诊断进行分类,提取甘肃省卫生系统疾病预防控制机构医院信息平台中2014—2017年兰州市城关区门诊数据建立数据库,描述性分析疾病谱。结果 2014—2017门诊病人就诊8 657 666人次,男女性别比为0.8:1,差异有统计学意义(χ~2=418.482,P<0.001);2014—2017年兰州市城关区门诊病人疾病谱未发生明显变化,主要系统疾病为呼吸系统疾病、泌尿生殖系统疾病、消化系统疾病、内分泌营养和代谢疾病、循环系统疾病;肿瘤是4年间门诊病人人数增加最快的疾病,年均增长率24.52%。结论呼吸系统疾病、泌尿生殖系统疾病、消化系统疾病、内分泌营养和代谢疾病是影响该地区居民健康的主要因素,并提示应注意监测肿瘤的发病;医疗卫生管理部门应结合当地疾病谱特点及规律,制定并实施相应的卫生保障措施,相关部门也应该根据疾病谱的情况进行有侧重的健康教育,更好地保障当地人群健康。Objective To understand the characteristics of disease spectrum change of outpatient cases in Chengguan district of Lanzhou city, and provide basis for the reasonable allocation of local medical and health resources. Methods The outpatient data of Chengguan district from 2014 to 2017 were collected from the Hospital Information System of Gansu Provincial Institutes of Disease Control and Prevention according to the ICD-10, and the disease spectrum was analyzed descriptively. Results There were 8 657 666 person times of outpatient cases from 2014 to 2017, and the proportion of the male to female was 0.8 ： 1 with statistical difference（χ^2=418.482, P〈0.001）. There were no obvious changes of disease spectrum of outpatient cases during the four years. The respiratory diseases, diseases of the genitourinary system and digestive system, endocrine nutrition and metabolic diseases, and the diseases of circulatory system were the main systemic disease. The outpatient cases with neoplasm grew rapidly during these four years with average annual growth rate of 24.52%. Conclusions The respiratory diseases, diseases of the genitourinary system and digestive system, endocrine nutrition and metabolic diseases, and the diseases of circulatory system are the major factors affecting the health of residents in this district. More attention should be paid to the surveillance of neoplasm. The protection measures and the emphasized education of health should be developed and implemented according to the characteristics and regularity of local diseases spectrum by the administrative department of health to protect the health of local people.
