拇手指再造术后应用静脉自控镇痛的临床研究

摘　　要：目的探讨拇手指再造术后经静脉途径应用PCA泵连续给药镇痛的疗效及护理观察重点。方法选择拇手指再造手术患者120例,ASAⅠ~Ⅱ级,随机分为2组。观察组术后使用静脉自控镇痛,对照组术后采取常规镇痛治疗。应用视觉模拟评分法(VAS评分法)评价镇痛效果,舒适评分(BCS)评价病人舒适程度,综合评估再造指体血运情况,再进行比较。结果观察组镇痛效果和优于对照组,血管危象的发生率低于对照组,均具有统计学意义(P<0.05)。结论拇手指再造术后应用静脉自控镇痛术后镇痛效果确切,可有效的改善再造指体血运,降低血管危象的发生,提高病人术后舒适度。Objective To analyze the effect of continuous intravenous administration of PCA pump for analgesia after thumb and finger reconstruction and the key point of nursing observation. Methods 120 cases of patients received thumb and finger reconstruction were selected, the cases were ASAI-II of anesthesia classification and they were randomly divided into two groups. Intravenous self-control analgesia was applied in the observation group and routine analgesia method adopted in the control group after operation. Visual analogue score（ VAS score） was used to evaluate the analgesic effect,comfort score（ BCS） was used to evaluate the patient＇s comfort. Comprehensively assess the blood supply in reconstituted fingers and compared. Results The analgesic effect in the observation group was better than those of the control group and the incidence of vascular crisis was lower than that of the control group,the differences were statistically significant（ P〈0. 05）.Conclusions Postoperative analgesia with intravenous self-controlled analgesia after thumb and finger reconstruction have exact effect,can effectively improve blood transport of reconstituted fingers and reduced the occurrence of vascular crisis,can improve postoperative comfort of patients.
