超细胸腔引流管在电视胸腔镜肺叶切除术中的应用效果分析

摘　　要：目的探讨超细胸腔引流管在电视胸腔镜肺叶切除术(VATS)中的应用效果。方法回顾性分析78例行VATS非小细胞肺癌患者的临床资料,其中实验组50例采用超细引流管辅助粗引流管胸腔闭式引流术,对照组37例采用常规改良胸腔闭式引流管。观察2组患者胸腔引流效果、疼痛评分。结果实验组1~4 d胸腔积液引流总量显著高于对照组(P<0.05),1~10 d总引流量及引流管留置时间无显著差异(P>0.05)。2组患者术后12 h VAS疼痛评分无显著差异(P>0.05),24~72 h静息及咳嗽时VAS疼痛评分有显著差异(P<0.05)。结论 VATS术后超细引流管辅助胸腔闭式引流更有效。Objective To explore the effect of super fine thoracic drainage tube on the treatment of patients with video-assisted thoracoscopic lobectomy （VATS）.Methods Clinical materials of 78 NSCLC patients with VATS were analyzed retrospectively.Fifty cases in the experimental group were treated with super fine drainage tube and thick drainage tube for closed thoracic drainage,and 37 cases in the control group were treated with conventional modified thoracic drainage tube.Effect of thoracic drainage and the scores of VAS were compared.Results The total drainage volume of pleural effusion in the experimental group from 1 st to 4th day was significantly higher than that in the control group （P ＜ 0.05）,but there was no significant differences in total drainage volume and indwelling time of drainage tube from 1 s,to 10th day between two groups （P ＞ 0.05）.There was no significant difference between the two groups in VAS score at 12 h after operation （P ＞ 0.05）,but the difference of VAS score from 24 to 72 h at resting and coughing were significant between two groups （P ＜ 0.05）.Conclusion After VATS,assisted super fine drainage tube is effective in closed thoracic drainage.
