健康人群颈动脉粥样硬化与臂踝脉搏波速度的相关性

摘　　要：目的探讨健康体检人群颈动脉粥样硬化和臂踝脉搏波速度(baPWV)及踝臂指数(ABI)相关性研究。方法对325例健康体检人群进行颈动脉超声检测,依据超声结果将其分为内膜正常组,内膜增厚组及斑块组,并进行臂踝脉搏波速度(baPWV)及踝臂指数(ABI)检测,结合相关血生化危险因素进行统计学分析。结果内膜增厚组及斑块组baPWV平均值均高于内膜正常组,差异有统计学意义(P<0.05)。内膜增厚组与斑块组baPWV值差异有统计学意义(P<0.05)。ABI值3组两两比较差异无统计学意义。内膜增厚组与斑块组总胆固醇、空腹血糖及血压水平均高于内膜正常组,且斑块组又高于内膜增厚组,差异有统计学意义(P<0.05)。baPWV与颈动脉内-中膜厚度、总胆固醇、空腹血糖、高敏C反应蛋白、同型半胱氨酸、收缩压及脉压呈正相关(P<0.05),且与颈动脉内-中膜厚度相关性最强。结论健康人群存在颈动脉粥样硬化时,可能同时存在baPWV升高,且随着颈动脉粥样硬化程度的进展,baPWV越高,呈正相关,ABI值变化则不显著。结合血生化检查有利于早期发现血管病变,为心脑血管病风险评估及早期干预提供了一定的临床依据。Objective To investigate the correlation between atherosclerosis and brachial-ankle pulse wave velocity（bw PWV） 、ankle-brachial index（ABI） in healthy people. Methods 325 healthy people classified into 3 groups according to the carotid ultrasound： normal, thickening and plaque group. Combined with baPWV、ABI testing and relevant biochemical risk factors were statistically analysis. Results The change of baPWV was significant in three different group（P〈0.05）, but ABI was not significant in three group. Total cholesterol（TC）、fast blood glucose（FBG）、blood pressure（BP）were significant in three group（P〈0.05）. bw PWV was related to TC, FBG, hs-CRP, Hcy, SBP, DBP, PP, and IMT respectively.Conclusion Carotid atherosclerosis is significantly related to baPWV, Combined with blood biochemical examination as early detection of vascular disease conducive for cardiovascular disease risk assessment and early intervention provides some clinical basis.
