摘 要:目的:评估酶联免疫斑点(Elispot)检测外周血中结核分枝杆菌性抗原特异性γ-干扰素(IFN-γ)在糖尿病合并肺结核诊断的应用价值。方法:选取糖尿病疑似肺结核患者共158例,其中确诊合并肺结核患者84例为观察组,糖尿病非肺结核患者74例为对照1组,选取同期肺结核患者90例为对照2组,三组患者均利用Elispot技术检测外周血IFN-γ水平、结核菌素皮肤实验(TST),分析Elispot技术的敏感性及特异性。结果:外周血Elispot技术检测观察组及对照2组较对照1组阳性率高,有统计学意义(P<0.05),观察组较对照2组阳性率比较无统计学意义(P>0.05),TST检测三组间阳性率无统计学意义(P>0.05)。外周血Elispot技术检测敏感度及特异度较TST比较有统计学差异(P<0.05)。结论:Elispot检测抗原特异性IFN-γ反应可有效诊断糖尿病合并肺结核,具有较高的临床价值。Objective: To evaluate the application value of enzyme linked immunostain(Elispot) for detection of mycobacterial antigen specificity in peripheral blood in the diagnosis of diabetes mellitus and pulmonary tuberculosis. Methods: Diabetes, a total of158 patients with suspected tuberculosis, among which 84 were diagnosed with tuberculosis patients as observation group, 74 cases of diabetes without tuberculosis patients as control group 1, choose the same period 90 cases of pulmonary tuberculosis patients as control group 2, three groups of patients are using Elispot technology to detect peripheral blood level of IFN-gamma, tuberculin skin test(TST), the sensitivity and specificity of Elispot technology analysis. Results: the peripheral blood Elispot technology test observation group and control group 2 than control group 1 positive rate is high, with statistical significance(P〈0.05), the observation group than control group 2 positive rate is no statistical significance(P〉0.05), TST test positive rate between the three groups have no statistical significance(P〉0.05). The sensitivity and specificity of peripheral blood Elispot technology were significantly different from TST(P〈0.05). Conclusion: Elispot detection of antigen-specific IFN-gamma reaction can effectively diagnose diabetes mellitus with pulmonary tuberculosis, which has high clinical value.
